Job Summary:
Provides screening, triage, risk assessment, treatment, and referral for consumers seeking mental health services. Provides consultation, education and information to families, providers, and community stakeholders regarding individual situations and treatment or community resources.

Education:
Required:
Masters Degree from an accredited Social Work Program and a Licensed Clinical Social Worker in the state of NC.

Experience:
Required:
Above average ability to use sound clinical judgment and communicate clearly in both written and verbal formats. Willingness to address opportunities for improvement. Ability to be extremely self-directed and able to communicate effectively with professional staff across many disciplines and programs. Ability to provide both brief and comprehensive psychiatric assessments.

Preferred:
Ability to compose and present psycho-educational lectures. Familiarity with regulatory and insurance reimbursement issues.
Experience in case management, knowledge of managed care practices, community resources, regulatory and insurance reimbursement issues
Experience in Behavioral Health or related field

Department Position Summary:
Coordinates telepsych consults on an ongoing basis and provides monthly stats to NCSTeP

Performs assessment/evaluation and documentation of clinical outcomes: This position provides direct client assessment which may include: triage, screening, assessments including risk and diagnostic assessment

Provides crisis intervention as needed

Petitions for involuntary commitment when clinically indicated

Provides brief therapeutic intervention, treatment planning, consultation, client and family education, diagnosis, sharing information with other agencies/providers and/or linkage to resources and services

Provides care coordination with MCOs, Public Health Department, providers, and community stakeholders regarding crisis stabilization and intervention as well as appropriate resources for referral

Completes diagnostic assessments, crisis plans and develops treatment plans

Documents, accurately and thoroughly, consumers' presenting problems, symptoms, treatment needs, recommendations and services accepted by the consumer

Shares and communicates documentation and treatment plan with the referring provider on an ongoing basis to assure continuity of care

Develops, coordinates and manages quality and regulatory compliance functions

Oversees and actively guides utilization review, medication monitoring, confidentiality of records and peer review in accordance with regulatory requirements

Develops/Assists with program policies which meet regulatory requirements

Works with Business Development and Marketing to develop an outreach plan to provide education to partners and community members regarding the program
Performs other duties as assigned that the employee is capable of performing

The Social Worker LCSW II Psych must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the dynamic of the patients served in the outpatient setting. The individual must demonstrate knowledge and respect of cultural differences and posses the ability to assess data reflective of the patient's requirements relative to his or her age-specific/cultural/social needs. Must demonstrate compassion and knowledge of addiction with a desire to educate/intervene to promote wellness. Assists the MD in overall management of patients with behavioral health needs to provide comprehensive care/support to patients in need. Collaboratively assesses, monitors, evaluates, and records quality outcomes to identify ongoing areas for improvement. Maintains adherence to department standards. Takes responsibility for departmental and organization objectives.

Maintains a pleasant and supportive attitude that fosters effective coordination of care and flow of patients. Serves as a liaison between patients and the providers while creating a safe, comfortable environment. Maintains strict confidentiality guidelines for behavioral health related information. Demonstrates sound work ethic and maintains standards of conduct. Must have the ability to plan time effectively, balance multiple tasks, work within stringent time frames, resolve problems, identify patient service trends, determine system improvements, and implement change. Must be flexible, self-motivated, and adapt to a changing, often intense environment.
