As a member of this team, you will play a vital role specific to the Behavioral Health Collaborative Care program. BHCC is an integrated care model which aims to improve behavioral healthcare access for patients with depression, anxiety and other common mental health diagnoses. While our collaborative care model is a joint effort between the Department of Social Work and the Department of Psychiatry, this position is located within the primary care setting and the reporting structure is through the Department of Social Work. The MSW is an integral member of a multidisciplinary team, which includes the primary care physician, psychiatric consultant, primary care site social worker, RN, PharmD, RD, etc.

Responsibilities*

  • You will be responsible for:
    • Psychosocial evaluation, symptom monitoring, records review, administration of screening tools, care management, and care coordination, as well as engaging patients to develop self-management plans.
    • Case consultation with the psychiatrist. This will require preparation, accurate and up-to-date patient registry and records maintenance, and the ability to make case presentations accurately and concisely.
    • Use of brief evidenced based therapeutic interventions, psychoeducation, safety planning, assisting family members in understanding and supporting the patient, and providing referrals to community resources.
    • Working directly with patients face-to-face, virtually, and by phone. A vast majority of patient contacts are complete by phone.
    • Ongoing collaboration with the patient's medical home team while drawing on social work experience and ongoing assessment of patient needs.


Required Qualifications*

  • A Master's degree in Social Work from a school accredited by the Council of Social Work Education
  • A current license to practice social work in the State of Michigan


Desired Qualifications*

  • Knowledge of or experience of working in a Collaborative Care Model (CoCM). https://mccist.org/about/cocm/
  • Experience with medical and mental health social work.
  • Experience consulting with psychiatric providers.
  • Efficient, accurate assessment of patient needs and effective case dispositions.
  • Skilled in developing rapport and effectively communicating with a diverse patient population.
  • Effective organizational skills, flexibility, self-motivation, the ability to effectively prioritize multiple competing tasks in a dynamic medical clinic.
  • Independent, sound decision-making is necessary for success in this position.
  • Effective collaboration with a large multi-disciplinary team.
  • Proficiency in keeping accurate and timely clinical records by computer.
  • Experience with brief, structured interventions (Motivational Interviewing, Behavioral Activation).
  • Familiarity with evidence-based interventions (e.g., CBT, PST, ACT, DBT) and psychopharmacology.


Work Schedule

This is full time, 40 hour/week position. The typical schedule is Monday-Friday with start and end times 8:00-4:30 or 8:30-5:00. However, there is the possibility for flexible scheduling and remote work.

Work Locations

This position will be split between Dexter Health Center and Domino Farms Family Medicine. This is currently a remote position but may be subject to change to include an onsite presence.

Underfill Statement

This position may be underfilled at a lower classification depending on the qualifications of the selected candidate. Advancement through the career ladder at the Expert or Clinical Scholar Level is required for placement in the MSW Senior Classification.

Background Screening
