Position Overview: Performs comprehensive psychiatric assessment and treatment of child or adult outpatients, 18 years of age and older, who have a wide variety of psychiatric illnesses so as to alleviate emotional distress and enable those patients to live more satisfactorily in the community.

Requirements
  • Develops and provides housing and entitlement resources to staff in an organized and understandable manner.
  • Manages housing/assisted living provider manual.
  • Track Adult PRP Supported Housing services for administrative reporting and policy change.
  • Review all applicants for Supported Housing Subsidy Program.
  • Complete and coordinate appointments with staff and patients for IRP Review Conferences in accordance with established standards.
  • Completes intake and rehabilitation assessments noting patient strengths, needs and goals in accordance with program and state policies and procedures.
  • Engages patient in rehabilitation process and ensures patient is oriented to program rules, services and patient responsibilities.
  • In conjunction with the patient, develops an Individual Rehabilitation Plan (IRP) in accordance with program and state policies and procedures.
  • Communicates and gives feedback to patient regarding the implementation and progress of goal achievement.
  • Provides recovery focused rehabilitation services centering on areas of independent living, activities of daily living, illness management, social support network building and community integration.
  • Completes clearly written screening and intake assessments according to established standards/time frames.
  • Completes discharge summaries and IRP conference reviews that reflect patient progress according to established time frames and procedures.
  • Completes needed documentation for billing purposes in accordance with established procedures and time frames.

Qualifications

  • Master's Degree in Social Work from an accredited school of social work.
  • Must be licensed by the Maryland Board of Social Work Examiners at the LMSW or LCSW- C level.
  • Minimum of 2 years of experience in providing clinical social work service, preferably, in a mental health/health care setting.
