Job Summary:

In this position, the VEPOP (Violence, Education, Prevention and Outreach Program) Social Worker provides comprehensive psychosocial assessment, trauma-informed care, and support to at-risk patients and families at University Hospital. Primary assignment in the Level I Trauma Center, Emergency Departments, and Med/Surg units, working closely with the Trauma Service and multidisciplinary team. In this position, the social work worker utilizes available data to identify and target at-risk populations, leverages community partnerships to make appropriate connections and referrals, and identifies individualized intervention strategies. In addition, the social worker addresses social determinants of health and works closely with a multidisciplinary team and community organizations such as the Office of Victims Services, the Division of Criminal Justice, and other related agencies to support the needs of the patients and families.

Minimum Qualifications:

M.S.W from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.

Work Days:

Monday-Friday, 8:30a-5:00p with occasional on call/holiday coverage on a rotating basis to meet operational needs.
