Job Summary
Investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.
Job Responsibilities
- Interviews patients, assesses priorities, and writes reports while managing complex caseload.
- Consults with team managers, statutory and voluntary agencies, and patient's relatives to ensure compliance with patient's treatment plan.
- Analyzes procedural manuals and communicates information to other hospital staff.
- Completes comprehensive assessments of patients.
- Assists patients/families with referrals to community agencies and provides education and resource information.
- Works in collaboration with the RN Case Manager and interdisciplinary team to ensure quality support of care and safety of discharge.
- Collaborates with Discharge Planning leadership on complex discharge planning issues and delays in service.
- Provides timely and concise documentation in the medical record, the assessment, concerns, needs, and discharge plan.
- Keeps plans up to date as patient progresses toward discharge.
- Supervises and/or trains other social workers, students, or interns as needed.
- Collaborates with the Medical Director, Director, Manager, and Supervisor of Case Management on patients who are not meeting the regulatory requirements for hospitalization.
- Provides initial and concurrent reviews for approval of hospital stay for all patients.
- Notifies patients when stay has been denied.
- Collaborates with the interdisciplinary team to ensure patient quality of care and safety of discharge.
- Bachelor's degree or higher in Social Work required upon hire
- No experience required
- LSW - Licensed Social Worker in the State of Kentucky required within 12 months of employment in this position OR
- LCSW - Licensed Clinical Social Worker in the State of Kentucky required within 12 months of employment in this position OR
- CSW - Certified Social Worker in the State of Kentucky required within 12 months of employment in this position
- Requires critical thinking skill and decisive judgment.
- Works under minimal supervision.
