Job Description

General Purpose of Job:

Performs general clerical duties associated with scheduling. To include all services, but not limited to scheduling for: caregivers for Home Hospice, Hospice House and Palliative care as well as patients for Home Hospice. Staffing decisions are made with supervision / support of appropriate manager.

Essential Duties:

This job description is intended to cover the minimum essential duties assigned on a regular basis. Caregivers may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.
  • Initiates and completes the preparation of schedules according to work loads in Hospice House and Home Hospice for both caregivers and patients.
  • Reviews staffing in MyTime (Kronos) in Hospice House and secures additional caregivers for coverage.
  • Generates visits in EMR (McKesson) system to allow for appropriate frequency of visits and notifies clinician when out of range.
  • Assists patients by determining needs, scheduling or canceling visits and maintaining schedule changes.
  • Secures and allocates additional clinical resources to fill daily / monthly scheduling deficiencies through use of per diem and contract agencies.
  • Daily review of clinic schedules to verify patients have been created and all visits have been added to the scheduling grid for the next business day (or week).
  • Maintains databases and updates as necessary
  • Accurate input of assigned patient information into the EMR (McKesson).
  • Obtain all necessary information from the patient to ensure proper scheduling
  • Proofread and check all completed work to ensure accuracy.
  • Notify the appropriate team member if any issues/errors are detected
  • Receives phone messages relevant to absences and communicates to appropriate Manager.
  • Collects relevant data and contributes information as appropriate
  • Resolves relevant issues through interpretation of appropriate policies and practices.
  • Orients new associates to scheduling process and specific policies related to these issues.
  • Provides clerical and substitute support as needed.
  • Organize and maintain confidential medical files and records.
  • Responds to inquiries on policies and procedures and/or refers questioner to correct staff person.
  • Complete other data entry tasks and related work as assigned/required.
  • Effectively communicate with staff to ensure accuracy.
  • Complete all related work as assigned.


Job Requirements

General Requirements

Certified Nurse Assistant

Work Experience

Minimum of 5 years' experience in a Hospice setting

Education

High School Diploma

Specialized Knowledge and Skills

Knowledge of data entry equipment and ability to set up computerized files required. Demonstrated excellent interpersonal/communication skills, verbal and written Demonstrated excellent keyboarding, extensive computer skills required Ability to assure confidentiality regarding sensitive materials Previous scheduling experience in health care setting (McKesson/Net Smart EMR preferred) Microsoft software experience preferred My Time (Kronos) scheduling experience preferred
