POSITION TITLE: Special Education/Case Manager

QUALIFICATIONS: NH Certification in General Special Education, knowledge of Special Education regulations and timeline, experience as a classroom and/or special education teacher.

REPORTS TO: Building Principal/Special Education Coordinator

SUPERVISES: Special Education Paraprofessionals

PERFORMANCE RESPONSIBILITIES

• Implement and monitor Individual Education Plans

• Evaluate students to identify needs and monitoring effectiveness of IEP

• Coordinate specialized evaluations for qualifying students

• Work with classroom teacher to ensure modifications are developed and implemented

• Coordinate the delivery of related services for the student

• Maintain and update a student's special education file

• Document and maintain all special education-related paperwork (WPNs, Meeting minutes, correspondence, etc)

• Train staff regarding expectations of needed modifications of identified students

• Coordinates and provides assisted technology

• Schedule, coordinate, and effectively prepare for special education meetings

• Facilitate IEP meetings using standard meeting protocols

• Communicate with parents as necessary to ensure families are aware of concerns and successes.

• Keep staff apprised of pertinent information regarding students on the caseload

• Verify that each IEP remains current and anticipates need to address IEP due dates

• Perform other such duties as may be required to ensure student success.
