We are currently looking for a Full Time Specialty Clinic Case Mgr/Infusion Nurse to join our Specialty Clinic team located at the Chickasaw Nation Medical Center.

Summary:

Provides routine patient care by utilizing and documenting the nursing process: including teaching patients and their families. Administers prescribed medications, IV fluids according to facility policies and procedures.

Responsibilities:
* Provides and advocates age and culturally appropriate, holistic patient care based upon facility mission statements and as outlined in policies and procedures
* Coordinates services a patient receives to include, but is not limited to, shared care planning, medication reconciliation, patient support and health education, and screening and tracking outcomes
* Assesses and recommends off-site sources for needed patient care, treatment and services not provided by the department or the organization
* Is a strong advocate and active participant in patient and patient family education i.e., diagnosis, medications and discharge instructions
* Administers prescribed medications, IV fluids and collects data on outside medications
* Collaborates with providers to screen and monitor patients with Hepatitis C and provides assistance to help patients adhere to Hepatitis C treatment regimens
* Implement interventions appropriate to the age-specific needs of the patient
* Facilitates consults, referrals, and form completion as requested and approved by the medical staff
* Participates in performance improvement activities to measure, assess and improve the quality of assigned work area
* Provides first class customer service - courteous and helpful to all internal and external customer types

* Maintain knowledge of and follow all laws, rules, regulations, policies and SOPs for assigned area, e.g. MICS, TICS, hospital standards, health codes, food sanitation, accounting principles, etc.

* Bachelors of Nursing

* Bachelor's degree from an accredited school of nursing, or written plan to obtain within 4 years of hire

Credentials:

* Registered Nurse
* Pass general background check
* Basic Life Support
* Must pass internal Medication Test

Essential:
* Current BLS certification or obtain within one (1) month of hire
