The Home Health Pediatric Speech Therapist assesses implements and evaluates a treatment program for home care patients to develop communication, feeding, swallowing, developmental, and cognitive abilities by utilizing appropriate speech therapy treatment modalities. The Speech Therapist coordinates care provision with the multidisciplinary team so as not to overwhelm the patient and family and to assure agreed upon outcomes for care.

  • Provides evaluations and ongoing assessments as a member of a multi-disciplinary team and provides trainings for staff and/or families;
  • Provides speech therapy services to Pediatrics who have language delays, feeding delays, cognitive impairments, trauma, injuries or other conditions requiring Speech Therapy which require in home therapy or natural environment services;
  • Demonstrate knowledge of normal and abnormal communication development; speech therapy intervention techniques; identification, evaluation and diagnosis of communication disorders;
  • Use and care of assistive devices used in speech and language therapy;
  • Responsible for completion of timely documentation including progress notes, reports, care plans, evaluations, and Medicaid authorizations;
  • Communicate effectively with families, members of the Individualized Family Service Plan (IFSP) team, and other health care providers regarding diagnosis and therapy.
  • Maintain punctual and dependable scheduled visits;
  • Participates in the development of the IFSP and makes recommendations/treatment plans for the overall care plan for the family;
  • Appraise parents' participation and instruct and train parents in programs individualized to their child
  • Adapt to different and changing environments and settings. Must be able to use and create different therapies according to the need of the patients and families, as a single plan may not work for all patients or families.
  • Able to work with all age groups.


Requirements

  • Must be licensed in the State of Colorado as a Speech Pathologist.
  • Must hold a master's degree in Speech/Pathology.
  • Accreditation: Must hold Certificate of Clinical Competence in Speech/Language Pathology from the American Speech/Language and Hearing Association or equivalent.
  • Proof of professional liability insurance if a contracted service; proof of health/immunization if a contracted service.
  • BLS certification required.
  • Certification for Colorado Early Intervention services.
