Status: Flex Full time (28-40 hours/week)The Transitional Care Unit (TCU) is a patient focused unit that bridges the patient care journey from inpatient care services to their next long-term living environment. TCU provides a multidisciplinary approach to nursing care, aligning a long-term care experience in a hospital-based unit, leading to a well-rounded experience for our patients & team members alike!

Responsibilities:

The Registered Nurse role, manages patient care, sets care standards, and collaborates with healthcare teams to ensure optimal outcomes. This role emphasizes leadership, communication, and commitment to the community and nursing profession.

- Leads or serves on teams to innovate patient care and promote a healthy workforce.
- Acts as a unit leader in clinical expertise, teaching, specialized resource nursing, or evidence-based practice.
- Provides constructive feedback and participates in nursing peer reviews.
- Drives change to enhance patient care quality and unit environment.
- Demonstrates advanced clinical assessment and analytical skills within Relationship-Based Care.
- Engages in shared governance and serves on hospital-wide councils.
- Passionately precepts students, graduate nurses, and new hires, supporting continuous professional development.
- Applies the nursing process to create a healing environment with holistic, patient-centered care goals.
- Practices strong communication, critical thinking, and comprehensive documentation to improve patient care.
- Promotes patient safety, quality, and satisfaction by aligning with organizational goals.
- Fosters a compassionate, healing patient experience while maintaining a healthy workplace and work-life balance.

Qualifications:

  • 2 years + experience strongly preferred
  • Long term care experience preferred
  • Previous charge nurse experience preferred
  • Ability to establish and maintain positive, caring relationships with managers, physicians, support staff, and patients/families.
  • Critical thinking skills necessary to exercise and to lead others in application of the nursing process.

Licensure, Certifications, and Clearances:
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Registered Nurse (RN)

*Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
