Job Description

General Purpose of Job:

The Case Manager MSW will resolve or minimize the social-emotional problems, resource deficits, and continuing care needs of patients, their families, and caregivers, who are served by Sparrow Specialty Hospital. In addition, the MSW will assist the patients, families, and caregivers to adapt to the social and emotional impact of their illness or disability, and facilitate, as far as possible, independent living in the community.

Essential Duties:
  • Performs a comprehensive assessment of the patient and family to identify issues which impact medical outcomes and living situation by:
  1. Performing focused psychosocial review of the patient's behavioral/cognitive functioning, emotional/mental status, adjustment to illness/injury, use or misuse of substances, and observation for indication of abuse/neglect.
  2. Performing an environmental assessment of the patient's support systems, finances, living situation, housing, legal concerns, and the patient choices and preferences.
  3. Reviewing the relationship between the patient's current medical conditions, physical functioning status, environmental factors and psychosocial condition/prognosis, and assist by managing the possible impact on medical outcomes, and continued care needs.
  • Provides individual or group supportive intervention to help patients and families adapt to the emotional impact of changes related to their illness/injury.
  1. Assisting the patient and families to gain insight to recognize and prioritize the problems needing to be addressed.
  2. Developing short and long-term intervention plans with the patient and family to address problems.
  3. Providing emotional support to help patients and families cope with the disruptions and distress associated with their illness/injury, and empowering them to regain control of their life and achieve independence.
  • As part of the interdisciplinary team, develops and implements a plan which addresses problems and needs identified in the assessment, and ensures continuity of care by:
  1. Meeting with each patient/family to complete case management assessment.
  2. Coordinates and conducts interdisciplinary family care conferences.
  3. Reviewing clinical information and engaging appropriate professional staff to communicate patient's progress towards discharge plans to patient/family.
  4. Assist in coordination of weekly Interdisciplinary Team meetings.
  5. Attends weekly Interdisciplinary Team meetings.
  6. Assisting patient and family access to appropriate resources and services based on a thorough knowledge of community resources and eligibility requirements.
  7. Advocating on behalf of patients/families with internal/external resources and payers for needed services, including negotiating with insurance companies for non-covered benefits.
  8. Providing or arranging education for patients and families to ensure continuity of care.
  9. Problem solving with patients and families to maximize their ability to follow preventive, remedial and rehabilitative recommendations.
  • Communicates with patients, families, members of the health care team, and others in a professional diplomatic and empathic manner.
  • Provides clear, concise, timely written documentation on the patient's medical record and on departmental records. Assigns accurate and complete billing codes, which are supported in written documentation.
  • Participates in the planning, implementation and evaluation of quality improvement activities within department, health system and/or community.
  • Compliance with Infection Control Standards, including Standard/Universal Precautions, Hand Hygiene and Personal Protective Equipment (PPE).
  • Performs other duties as assigned.
  • Maintains knowledge of current trends in area of expertise and provides evidence of current competencies.
  • Demonstrates knowledge and respects patient, service provider, organizational confidentiality and HIPAA Security procedures and protocols as defined under the HIPAA Privacy and Security Procedures
  • Supports the development of self and other associates and students through mentoring, preceptorship and peer review.
  • Accountable for professional development.
  • Models Hospital customer service behavioral standards.

  • Utilizes current evidence and research in practice.


Job Requirements

General Requirements

ACSW preferred BCLS preferred Registered or certified in Michigan.

Work Experience

Experience in both outpatient and acute care setting preferred.

Education

Master of Social Work Degree from an accredited School of Social Work,

Specialized Knowledge and Skills

Demonstrated excellent communication skills including relationship building, negotiation, persuasion, marketing, and advocacy. Demonstrated ability to handle many tasks at one time, organize, set priorities, work independently, and manage time. Demonstrated ability to work in stressful situations and manage conflict. Knowledge of health care financial environment, health system and community resources.
