Job Summary

The Staff Pharmacist prepares medications by reviewing an interpreting physician orders, detecting therapeutic incompatibilities. Complies with the State and Federal drug laws as regulated by the State Board of Pharmacy, the Drug Enforcement Agency, and the Department of Public Health. Provides pharmacologic information by answering questions and requests of health care health care professionals. Completes pharmacy operational requirements by organizing and directing technician's workflow, verifying their preparation and labeling of pharmaceuticals, performing order entry, as well as charges and inspections. Controls medications by monitoring drug therapies and advising interventions. Dispenses medications by compounding packaging and labeling pharmaceuticals. Prepares compounded sterile products and chemotherapy within a USP 797 compliant clean room.

Job Requirements

Minimum Education
  • Pharm D degree (or B.S. Pharmacy degree prior to 2006) from an accredited Pharmacy School

Minimum Work Experience
  • Clinical background to help develop programs preferred


Required Licenses
  • Pharmacists, Current MA licensure required


Required Skills
  • Good communication skills to relate with medical and nursing staff required

Physical Requirements:

  • Enters/verifies all medication orders correctly into patient medication profiles in compliance with hospital formulary. Interprets medication orders accurately Maintains accurate and complete patient medication profiles
  • Monitors drug therapy regimens for contraindications, drug drug interactions, drug-food interactions, allergies and appropriateness of drug and dose
  • Demonstrates knowledge of appropriate standards, policies, protocols, procedures and practices in a manner that demonstrates baseline knowledge
  • Proficient in dosing/monitoring of medications with an established protocol . Provides double checks as needed on pharmacokinetics consults to detect calculation errors and assess outcome goals
  • Documents interventions in the Pharmacy module that advocates for IV to PO conversions ,dose optimization , dose adjustments, therapeutic duplications, drug interactions, allergy conflicts and other intervention groups for antimicrobial, other medication therapies or therapeutic recommendations.
  • Supervises the manufacture and distribution of medications and IV solutions by the Pharmacy Technicians to ensure medications are delivered to the patient care area staff at the scheduled times of administration.
  • Contacts prescriber about questionable orders and provides information on the correct therapy, noting formulary alternatives when indicated.
  • Educates patients and clinicians regarding medications.
  • Collects data for DUR and ADR programs.
  • Prepares chemotherapeutic medications according to the program so that patients receive the correct dose and medication at the scheduled time.
  • Systematically gathers and evaluates relevant information and data from the patient interview and/or all the patient information sources for the purpose of maintaining an accurate patient profile
  • Able to recommend appropriate antibiotic therapy based upon culture and susceptibility data or suspected infection
  • Demonstrates an understanding and completes all job functions in accordance with the five patient rights
  • Follows hospital policy in reporting medication errors and adverse drug reactions
  • Assists physicians and nurses with medication reconciliation
  • Follows all policies and procedures related to inventory management and control
  • Promotes critical pathways that encourage the selection of antimicrobials and antimicrobial combinations that delay or prevent the emergence of resistance
  • Follows practices that encourage selection of the antibiotic regimens with the narrowest spectrum of activity based on culture and sensitivity results
