Job Description

SUMMARY OF ESSENTIAL DUTIES AND RESPONSIBILITIES:

To provide program management support for community-oriented partnerships and initiatives, including the Enhanced Health Services at Street Homeless Engagement Sites Program project with NYC DHS, and other potential future initiatives. Reporting to the Director of Street Health Outreach & Wellness, the role sits within the Office of Ambulatory Care at NYC Health + Hospitals, and interfaces with colleagues internal to H+H, as well as externally with NYC Department of Homeless Services (DHS), DHS-contracted Safe Haven, Shelter, and Drop-In Center operators/providers, and other community-based organizations.

Responsibilities:
• Manage implementation of the Enhanced Health Services at Street Homeless Engagement Sites Program project plan, in partnership with DHS counterparts and H+H stakeholders
• As needed, develop project plans and implementation tactics for additional community-oriented initiatives
• Build relationships with H+H facility leaders, and engage with these stakeholders as part of initiative implementation and on-going program management
• Organize and coordinate program-specific staff trainings for SHOW and Enhanced Health Services at Street Homeless Engagement Sites Program staff
• Assist with program financial plan development and management
• Coordinate with Central HR, Facility HR, and facility-based supervisors to support the hiring process for project-affiliated staff
• Other program and project-based responsibilities as determined by SHOW and Ambulatory Care leadership

Minimum Qualifications

Educational and/or experience requirements will vary requiring diverse kinds and levels depending on the content, complexity and variety of project. Compensation levels will correspond to the requirements developed for the designated project.
In no event will the acceptable minimum be below:
1. A Baccalaureate Degree from an accredited college or university in a specialization discipline consistent with project content; and,
2. Four years of substitutable and relevant health care experience related to the objectives of the project; or,
3. A satisfactory equivalent combination of experience, training and education.

Department Preferences
• 3-5 years project or program management experience
• Experience with matrixed organizations and team structures
• Strong communications and presentation skills
• Project implementation experience
• Ability and willingness to travel to multiple sites and work locations within NYC
