With a commitment to patient- and family-centered care, the Nurse Navigator RN uses the nursing process in ways that values the uniqueness of each individual and addresses the physical, psychological, emotional, and social needs of the diversity of patients served by Jefferson Health. The Nurse Navigator RN utilizes advanced nursing skills and knowledge to comprehensively coordinate care for a defined patient population to facilitate patients through the continuum of care. The Nurse Navigator RN collaborates with physicians, nurses, social workers and other health team members to expedite medically appropriate, cost-effective care. Responsibilities include risk assessment and management of clinical populations, discharge planning, and utilization management. The Nurse Navigator RN acts as a patient advocate and helps patients and their families understand their health status and treatment options while ensuring that the patient receives the assistance necessary to follow through with their personal treatment plan.

Essential Functions
  • Interacts with co-workers, patients, their families/designated care partner(s), and other staff consistent with the values of Jefferson.
  • Integrates evidence-based practice, clinical expertise, patients, their family/designated care partner(s) preferences and cultural beliefs and values for delivery of optimal health care.
  • Partners with patients, their family/designated care partner(s) in identifying learning needs and priorities and provides education in ways that is useful, understandable and affirming.
  • Advocates for the patient and family throughout the entire episode of care. Functions as a resource to facilitate communication among the healthcare team and patient/family.
  • Facilitates patient progression throughout the patient's care continuum.
  • Reviews patients' status to identify issues requiring priority interventions or attention which may impact the patient's treatment plan.
  • May facilitate daily patient Plan of Care Huddle Rounds functioning as the Patient Progression Leader or supports the identified Patient Progression Leader.
  • Monitors patient's progress intervening as necessary to ensure patient progresses efficiently.
  • May provide oversight related to specific documentation requirements for patient populations to ensure consistency, accuracy and core measures are met by physician and nursing staff
  • Serves as a key facilitator in identification of patient/family education needs and utilization of systems/materials to best meet those needs across the continuum for a defined patient population
  • Participates education/training for member of the healthcare team

Qualifications

Minimum Education and Experience Requirements:

Education:

Graduate of an accredited school of nursing required; BSN or completion of BSN or appropriate advanced degree within 3 years of hire strongly preferred.

Experience:

Minimum of 3 years' experience in the specialty area being recruited for required.

Minimum Certifications, Registration or License Requirements:

Current licensure as a Registered Nurse in the state of practice; BLS required based on organizational guidelines/polices. ACLS and PALS as required by department. Certification in specialty preferred

If Certification is required on a specific unit; certification must be obtained within six months of hire date.

Conditions of Employment
