We are seeking compassionate and skilled Registered Nurses to join our dynamic Hospital at Home team in College Station, Texas. As a Hospital at Home Registered Nurse, you will have the unique opportunity to provide personalized care to patients within their homes. You will play a crucial role in assessing clinical and environmental needs, fostering a healing environment, and supporting patients and their families through education and emotional support.

KEY RESPONSIBILITIES

• Provide high-quality acute care services to patients enrolled in the Hospital at Home program.

• Conduct comprehensive assessments and develop individualized care plans tailored to each patient's needs.

• Spend additional time at bedside assessing clinical and environmental needs and supporting an overall healing environment.

• Act as a key educator and provide ongoing support to patients and their families, ensuring they have the necessary resources and understanding to manage their care at home.

• Collaborate closely with interdisciplinary teams to coordinate care and optimize patient outcomes.

Qualifications - External

QUALIFICATIONS

EDUCATION - Grad of an Accredited Program

MAJOR - Nursing

CERTIFICATION/LICENSE/REGISTRATION - Registered Nurse (RN)

Basic Life Support (BLS): BLS or BLS within 30 days of hire or transfer.

Job Summary

As a Registered Nurse, will oversee patient care provided by the healthcare team using a professional practice model. Your responsibilities include patient assessment, recognizing health issues, creating care plans, and assessing patient responses.

What You'll Do

  • You, as an RN, will conduct detailed clinical assessments, prioritize patient and family needs, create, implement, and evaluate care plans. Adjust plans to reach desired outcomes.
  • Building relationships, managing discomfort, staying professional are vital in therapeutic connections with patients and families.
  • Creating care plans reflecting values aids in resolving issues.
  • You will help educate patients, families, nurses, and community members to empower them with knowledge for better healthcare choices. Assess and document learning needs regularly.
  • Work with colleagues and community to create a care plan. Delegate tasks carefully and follow up promptly.
  • You'll use different strategies for problem-solving with patients, families, and staff. Recognize limitations and prioritize safety, effectiveness, and efficiency in patient care planning and delivery.

Qualifications

  • Grad of an Accredited Program
  • Specialized field of study should be nursing.
  • A current and valid nursing license.
