Summary:
Provides leadership, management and direction for the Behavioral Health social workers and Transitional Care Coordinators to ensure that there is high quality, cost-effective delivery of social work services and discharge planning, which meets patient needs, hospital needs and regulatory requirements of Ohio MHAS (Ohio Department of Mental Health and Addiction Services), and Joint Commission. Manages social work and transitional care staff, monitors performance and hospital need and evaluates progress towards goals.
Demonstrates responsiveness to changing health care needs of the patient population and changes in insurance contracts. Holds key position as a resource for the TCCs for discharge planning needs. . Manages Behavioral Health Transitional Care and Social Work staff, monitors performance, staffing, and evaluates progress towards goals.

Minimum Qualifications:

Formal Education Required:
-Masters Degree in Social Work.
-Current Ohio LISW License.

Experience and Training Required:
-Five (5) years experience of behavioral health social work, preferably in an acute inpatient setting with one year of supervision.
- Has a broad understanding of Utilization Management concepts and the insurance industry.
-Has understanding of work involved in transitional care management.
-Management experience preferred.
-Knowledge of and experience in Quality and Performance Improvement.

-Sound knowledge of community resources/agencies.
-Ability to communicate effectively, accept and institute change and work independently as well as collaborate with others.
-Population Specific Competency: Ability to effectively interact with patients/customers with the understanding of their needs for self-respect and dignity.

Level of Physical Demands:
-Sedentary: Exerts up to 10 pounds of force occasionally and/or a negligible amount of force frequently.
