TPAC Coordinator

Description

TPAC Coordinator is responsible for managing the TPAC residence from pre-admission to discharge. Assess, plan, implement, coordinate, monitor, and evaluate options and services with a primary goal of providing a safe transition from acute care to home. Will develop a care plan including long term and short term goals for improvement. Assist patients in the process of navigating post-acute care .

Position Responsibilities:

• Coordinate Interdisciplinary Team's work along with the Resident Assessment Instrument process assuring the accuracy, timeliness and completeness of the MDS and Interdisciplinary Care Plan.
• Establishes the schedule for completion of the Medicare required assessments. Follows a multi-disciplinary model to identify patient needs and appropriate reimbursement.
• Completes and/or oversees completion of all Medicare MDS assessments.
• Completes accurate coding of the MDS with information obtained via medical record review as well as observation and interview with facility staff, resident, and family members.
• Participate in the admission and discharge process of prospective patients in terms of their nursing needs and appropriate placement; Facilitate problem-solving for complicated admissions.
• Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations.
• Monitor Medicare assessment schedules and nursing documentation to ensure accuracy and timely submission.
• Review and maintain all required state and federal records as it pertains to MDS assessment for residents' treatment.
• Coordinate interdisciplinary comprehensive Care Plans for resident treatment.
• Monitor and document the cost effectiveness of treatment provided.
• Review potential referrals for skilled needs and cost effectiveness of an admission to the facility.
• Perform case management and/or nursing duties as needed.
• Responsible for staff and physicians training regarding post-acute care requirements/care.
• Fiduciary Responsibility. TPAC Coordinator is a fiduciary of the organization, and is obligated to act in the best interest and for the benefit of the organization at all times.

Requirements

• Must have a current, unencumbered RN license to practice in the state of Colorado (compact acceptable).
• 2+ years of experience with MDS in a long term care environment is preferred.
• Must be able to relate information concerning a patient/resident's condition.
• Able to follow all applicable policies and procedures as well as current federal, state and local standards, guideline and regulation that govern swing bed facilities.
• Must be able to react to emergency situations appropriately when required
