Job Summary

Provide coordination of the various functions associated with CAMC's kidney transplant program, including pre-transplant recipient evaluation, coordination of total patient care, post-transplant follow-up, record maintenance, and coordination of consultative services.

Responsibilities

• Responsible for the maintenance of all appropriate records for kidney transplant patients. • Maintains an updated list of all patients waiting for renal transplantation. • Gathers and submits data required by various regulatory health agencies. • Accountable for coordinating total patient care during the transplant patient's hospital stay. Responsible for coordinating patient and family education. • Accountable for making daily hospital rounds on all post transplant patients and patients hospitalized for adjunctive procedures preliminary to transplantation. • Accountable for providing transplant patients with detailed educational instruction prior to hospital discharge and discharge planning with other disciplines. • Accountable for providing patient follow-up after transplantation, answering questions, reviewing daily labs and procedures, and reports to appropriate physician. • Accountable for organizing the schedule of the multidisciplinary clinic where transplant patients are seen in follow-up, participating in the follow-up evaluation. • Participate in ongoing transplant research, including all phases of clinical trials while maintaining appropriate documentation. • Serve as a liaison with the referring Physician. • Serve as a resource for transplant-related topics, providing education both formal and informal. in various transplant-related techniques. • Perform the pre-transplant evaluation of potential kidney recipients and living donors. • Schedule patients for operations preparatory to transplantation as well as for the transplant operation. • Review and/or interpret preliminary laboratory test results, notifying the appropriate Physician of the results. • Maintain an updated list of all patients who have been or are in the process of being evaluated for kidney transplantation. • Responsible for ensuring CMS/UNOS/DNV standards are met for transplant services. • Independently identifies areas for QI/PI and develops, implements, and evaluates using proper QA measures.

Education

• Bachelor's Degree (Required) Experience: 2-3 years of nursing experience Substitutions: Associate Degree with 5 years of Nursing Experience may substitute for a Bachelor's Degree
