The role you'll contribute:

Participates in patient assessment, counseling, support, and resource linkage to pre and post-transplant patients and living donors. Works closely with physicians and other medical staff to aid patients in understanding the donor process, rights and facilitating decision making, as well as providing a variety of other case work services in acute care areas, outpatient, and in the community. Assumes personal accountability for organizational mission and core values.

  • Assesses patient and/or donor's psychosocial, cultural and spiritual needs through observation, interview, review of records and clinical data. This includes assessment of family relationships, support system, mental health history, personality traits and coping skills.
  • Conducts informal and formal family and nursing conferences to ensure most efficient coordination of health care and related services.
  • Evaluates patient and family response and intervenes in a timely manner to ensure optimal patient outcome. Serves as a patient advocate and anticipates needs of patient, family, and staff.
  • Conducts individual or group therapy sessions as needed. Assesses patients' progress and modifies or extends treatment program accordingly.


Qualifications
The expertise and experiences you'll need to succeed:
  • Master's Degree Social Work
  • One year of related clinical social work experience
  • Demonstrates clinical and ethical professionalism, conflict resolution and critical thinking skills.
  • Licensed Clinical Social Worker - Current Colorado State License as a Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW) with being fully licensed (LCSW) within 2 years.
