Job Summary

Reporting to the Clinical Director, and under the general direction of the Medical and Surgical Dirctors of the Mechanical Cardiac Assist Program, assists in the development and growth of the Ventricular Assist Device Program. The VAD Coordinator will provide clinical expertise for patients with heart failure requiring ventricular assist device placement through all phases of care in the inpatient and outpatient setting. This role provides care to adult patients aged 18+.

Essential Functions:
  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson
  • Meets all standards of the Patient Satisfaction Initiative Criteria in order to make Jefferson Abington Health (JAH) the hospital of choice for Patients and Employees
  • Responsible for clinical and technical support of patients regarding evaluation, implant management, post follow-up and clinical training. Coordinates patient care through medical procedures, ongoing education, inpatient and outpatient care delivery and follow-up.
  • Manages the MCS program research protocols and documentation, including Intermacs in coordination with the research nurse coodinrators. Provides maintenance of Joint Commission certification and tracking of outcome and quality standards
  • Provides on-call coverage for inpatient and outpatient VAD patients or for potential procedures in the OR or other procedural areas.
  • Provides care to advanced heart failure and VAD patients functioning in an expanded roll using the nursing process and integrating acts of medical diagnosis and therapeutics. Records physical findings and formulates plan based on patient's condition to maintain complete and accurate patient medical record in the physician office and hospital.
  • Acts as a liaison between patients, families, hospital staff and referring physicians. Participates in community outreach and educational lectures
  • Performs other related duties as assigned


Qualifications

  • BSN registered nurse and/or MSN, advanced practice provider required
  • 2+ years of experience in heart failure and/or cardiac critical care required and minimum 2+ years of VAD program experience preferred
  • Word, Excel, PowerPoint, Outlook, Epic
