Job Summary

The core function of this position is coordinating all aspects of the VAD process to ensure a good patient experience. As a self-starter in this new program, the selected candidate enjoys identifying areas for process development, developing policies and protocols, and taking the lead to reach solutions. Key to success in this role is the ability to provide a good patient experience, and to effectively collaborate with other team members at various levels and disciplines in/outside the organization.

Responsibilities

The VAD Program Coordinator is a critical member of the multi-disciplinary team for the advanced heart failure/ventricular assist device patient population. The VAD coordinator will manage all aspects of the VAD patient's experience, from facilitating referral to the program, to evaluation, through implantation, hospital discharge, and ongoing follow up. The VAD coordinator interfaces with many departments and agencies internally and externally. This responsibility includes significant collaboration with physicians, cardiac departments, outside medical facilities, patients and their families and other services.

Knowledge, Skills & Abilities

Patient Group Knowledge (Only applies to positions with direct patient contact) The employee must possess/obtain (by the end of the orientation period) and demonstrate the knowledge and skills necessary to provide developmentally appropriate assessment, treatment or care as defined by the department's identified patient ages. Specifically the employee must be able to demonstrate competency in: 1) ability to obtain and interpret information in terms of patient needs; 2) knowledge of growth and development; and 3) understanding of the range of treatment needed by the patients. Competency Statement Must demonstrate competency through an initial orientation and ongoing competency validation to independently perform tasks and additional duties as specified in the job description and the unit/department specific competency checklist. Perform related duties as assigned.
