The Vice Chair and Enterprise Lead for Community Medicine will provide academic and health system leadership in meeting the needs of communities affected by poverty, racism, and social injustice. The candidate will drive regional health equity and population health through building systems that address social needs and health equity for the regional population for whom Jefferson provides comprehensive, longitudinal care through primary care attribution and empanelment. This population comprises 560,000 primary care patients in the greater Philadelphia region, including over 100,000 people on Medicaid and more than 20 community-based sites in low-income neighborhoods. The candidate will report to the Chair of Family and Community Medicine/ Enterprise Chief of Primary Care and to the Executive Vice President for Health Equity and Community Engagement.

Requirements: Physician with at least 5-10 years' experience working in clinical care in community-based settings serving low-income people, demonstrated success in developing and implementing clinical models and initiatives, and demonstrated success working effectively with community coalitions and partners.

The successful candidate will work in close partnership with EVP Health Equity, Chair/Enterprise Chief of Primary Care, and other key leaders to:
  • Develop and execute on strategy for social needs complexity across the clinical care of populations, with emphasis on excellence in care delivery for attributed and primary care empaneled populations
  • Ensure high-quality implementation of standard approach to social needs across Jefferson's attributed/empaneled population
  • Identify areas of disparate health process and outcomes in clinical management of the attributed/ empaneled population, and partner with clinical and population health leaders to define and address focus areas.
  • Create complex care models for people with high-intensity social needs, focusing on primary care as foundation of continuous, comprehensive care, and working with clinical, operational, and financial leadership to build sustainable models.
  • Grow grant portfolio aligned with defined needs and priorities of Jefferson's Community Medicine sites, including collaborative grants with community organizations.
  • Collaborate with Jefferson Health Plan and other insurers in developing high-quality models of community-based care.
  • Ensure community-centeredness in new initiatives for community-based clinical care
  • Model and maintain a work culture of inclusiveness, transparency, and support for patient-facing teams. Listen to and serve as a voice for clinical teams working in Jefferson's community-based primary care sites.
  • Support the progression of colleagues' academic faculty careers across departments, and of professional careers across regions, through mentorship, sponsorship, and opportunities to engage in organizational initiatives
  • Promote opportunities in Community Medicine for students, medical residents, and other learners
  • Identify research and evaluation opportunities within Jefferson Health and in partnership with community-based organizations
