Summary:


This individual is responsible for skilled services under the supervision of a Registered Nurse (RN) and in accordance with the Plan of Care and with the agency policies; prepares clinical progress notes, assists the RN in performing specialized procedures, prepares equipment and materials for treatments and assists the patient in learning appropriate self-care. The RN delegates skilled nursing functions after evaluating the complexity of the nursing activity and the level of problem solving required.

Responsibilities:


Assists with coordination of patient care with other members of the home care team.

P rovides problem solving skills to assist in coordination of daily scheduling.

P articipates in patient care conferences and Team meetings,allowing for the exchange of information in both formal and informal networking systems.

Provides skilled nursing care to patients in accordance with the Plan of care under the supervision of an RN.

P erforms delegated nursing activities which are appropriate to the LPN's preparation and experience as evidenced by skills checklist and in accordance with agency policies and program guidelines.

C ontacts providers for clinical orders,referrals and treatments as indicated.

D ocument and implement direction as appropriate.

I mplements the plan of care, following the RN's initial assessment and development of the plan of care , utilizing practice standards.

C onferences with the RN on a regular basis, collects specific assessment data, participates in the planning,evaluation and modifying of the nursing care plan as appropriate.

P rovides for emotional and physical comfort and safety of patients.

R ecognizes and understands the effects of social and economic problems with patients.

W orks with Home Health Aides (HHA's) and other disciplines as appropriate.

M aintains records in accordance with agency requirements.

P articipates regularly in agency staff meetings, team meetings and inservices.

P articipates on agency committees or special task forces as assigned.

P articipates in development , revision and implementation of policies and procedures.

A ssumes responsibility for personal and professional growth.

D emonstrates excellent customer service.

C ontributes to organization success targets for patient satisfaction and not operating margin.

M aintains confidentiality of patient and agency information .

R esponsible for special projects and other duties as assigned.

EDUCATION/EXPERIENCE

C urrent licensure with the Michigan State Board of Nursing.

B LS certification .

G raduation from a school of practical nursing NAPNES recognized.

T wo years of nursing experience following graduation or at the discretion of the Program Services Supervisor.

H omecare experience preferred.

K NOWLEDGE/SKILLS/ABILITIES

K nowledge in current nursing practice, ability to relate positively to patients, families and co-workers.

A bility to read, write, speak and comprehend English.

S econd language desirable but not required.

K nowledge of basic computer programs.

V alid Michigan Driver's License.

E xcellent communication skills, both verbally and written.

C omfortable with the use of the computer program, and other software systems used within the VNA system.

D emonstrates problem solving skills.

A bility to type.

N ear visual acuity and functional hearing required.

S uccessful ability to independently access any type of a private residence.

A bility to lift greater than 20 pounds.

R eliable transportation and proof of current Insurance.

W ORKING CONDITIONS/PHYSICAL DEMANDS

A bility to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards .

C onstant talking, hearing tasting/smelling, near vision, midrange vision, far vision, depth perception, visual accommodation, color vision and field of vision.

F requent sitting, twisting, reaching, handling, fingering and feeling.

O ccasional standing, walking, lifting, carrying, pushing, pulling climbing, balancing, stooping, kneeling, crouching and squatting.

O ccasional lifting 0-100 lbs.
