Summary:


This individual is responsible for augmenting the other home health care services provided by the agency. The clinical social worker prepares social histories, treatment plans, treatment goals, progress notes and discharge notes for home care, palliative care and hospice clients and their families. The MSW assists staff in understanding significant psychosocial factors related to or affecting a client's health status or aids in referrals to social agencies for problem resolutions. This position is responsible for the facilitation of Hospice and Palliative Care Bereavement services and provides supervision of the BSW as required.

Responsibilities:


Assess social, emotional, financial, spiritual and support system status for hospice, palliative care and home care patients and families.

A ssess special needs related to cultural diversity and communication patterns.

A ssess potential risks including suicide, fraud, and abuse.

A ssess for spiritual need .

A ssess environmental resources for safety.

A ssess need for counseling related to pathological grief. Identify obstacles to compliance.

E valuate for and assist with placement into long-term care when appropriate.

I nterprets patient response and collaboratively identifies interventions to maximize positive outcomes.

A ctively prepares and participates in accrediting agency surveys.

P rovide social casework for hospice, palliative care and home care patients and families.

A ssist with provision of bereavement services.

A ble to assume role of Bereavement Coordinator supervising bereavement program and working with Hospice/Palliative Care Manager to maintain compliance with regulations as directed.

P rovide support to other VNA departments where social work support is needed.

Serve as a patient and family advocate.

E nsure continuity, timeliness, and appropriateness of care for designated caseload.

E nsure timely completion of plan of care and communication with IDT.

I nvolved in the process for delivering the Advanced Beneficiary Notice of Non-Coverage.

B uilds relationship within the organization and serves as a resource to the healthcare team related to community resources.

E nsures timely completion and reporting of all documentation.

R eviews cases for risk management issues and intervenes when indicated.

F acilitate community grief support group.

P rovide assistance with Advance-Directives and funeral planning.

A ssist other caregivers (RN's, MSW) in explaining available community, hospice, palliative care or home care services.

P articipate in area social service groups as indicated.

P rovide presentations to local community groups and staff on a variety of topics.

D emonstrates age specific competency in all age groups: Infant, Pediatric, Adolescent, Adult and Geriatric .

R epresents agency in a professional manner.

C ompletes annual education and compliance per VNA Policy and agency accreditation guidelines.

M aintain patient confidentiality according to all applicable laws, policies and regulations.

P repares social histories to augment existing services on request.

A ssists in the identification of unmet needs in the community and makes recommendations to Home Care Nursing Manager and Hospice/Palliative Care Manager.

C ommunicates status, problems, referrals, or assistance provided to the nurse on a regular basis.

A ssesses a patient's and family's situation and refers financial concerns to community social service agencies.

S erves as a liaison to interpret available social services resources to agency staff or available health care services to social services agencies.

M aintains chart records and completes documentation in accordance with agency requirements and procedures .

P articipates in bereavement follow-up, as a member of the hospice and palliative care team.

EDUCATION/EXPERIENCE

Master's degree in social work from accredited institution.

Current State of Michigan license required.

T wo years' experience in a medical/health care related setting.

KNOWLEDGE/SKILLS/ABILITIES

Knowledge of hospice, palliative care and home care philosophy and services.
