Conditions of Employment

W-4 Form; 5. Washington State Department of Retirement Status Form; 6. Immunization History (if required) 8. Hepatitis B Training (if required); BY SELECTING "YES," YOU AGREE TO PROVIDE/COMPLETE THESE REQUIRED ITEMS.
• I authorize Ferndale School District to make any investigation of any personal, educational vocational, or employment history. I further authorize any former employer, person, firm, corporation, educational or vocational institution or government agency to provide the District with information they have regarding me. I hereby release and discharge the District and those who provide information, from any and all liability as a result of furnishing and receiving this information.
• I certify that answers given herein are true and complete to the best of my knowledge. In the event of employment, I understand that false or misleading information, including the omission of material information, given in my application or interview(s) may result in discharge. I understand that I am also required to abide by all policies and regulations of the District.
• Do you have or are you willing to obtain, a valid Washington State Driver's License and Class B, CDL with appropriate endorsements?
• Do you have, or are you willing to obtain, a valid school bus driver authorization and five-year complete school bus driver abstract?

• Do you have at least five (5) years prior driving experience?

• You will be asked to provide a Driving Abstract from the Department of Licensing. If you have had any traffic citations in the last five years which could include DUI, reckless, negligent, speeding or hit and run driving, please explain in detail date(s) and incident(s).

• As required by Chapter 29, Laws of Washington 2004 and Chapter 28.400.RCW, do you authorize your current and past employers to disclose to the Ferndale School District, copies of all documents in your current and/or previous employer's personnel, investigative, or other files relating to sexual misconduct and further release your current and/or past employers, and employees acting on behalf of those employers, from any liability for providing such information?
