Permanent Transfer Candidate: Current NYS employee with one year of permanent or contingent-permanent, competitive, service as a Health Care Surveyor 2, G20.

Non-Competitive: Possession of a license and current registration as a registered professional nurse in New York State; and two years of post-licensure, specialized professional administrative or professional clinical experience.

Substitution: A bachelor's or master's degree in Nursing can be substituted for the one year of required general nursing experience

Preferred Qualifications: Must have strong clinical knowledge; interpersonal, problem solving, decision-making and time management skills; work well independently and as part of a team and have the ability to communicate effectively with all internal and external stakeholders. In addition, the ideal candidate must have excellent writing and editing skills and be able to effectively interpret regulations governing Home and Community Based Services and Program of All-Inclusive Care for the Elderly (PACE). Home Care Health Care experience a plus.

Duties Description The Health Care Surveyor 2 in the Division of Home & Community Based Services Program will be responsible for routine surveillance and investigation of complaints and incidents of licensed home care services agencies, certified home health agencies, hospices, and Program of All-Inclusive Care for the Elderly (PACE) to ensure that care provided meets standards of practice and complies with State and federal statutes and regulations. The surveyor evaluates the care and services provided to individuals in their homes, assisted living facilities, adult homes, hospice residences, nursing homes and PACE programs/facilities. The surveyor will conduct surveys in a professional and objective manner in accordance with the applicable state and/or federal regulations, and policies and procedures. The surveyor will complete required survey activities in accordance with the established time frames, either independently or in collaboration with others. They incumbent will also gather accurate and complete information from all possible sources of information (reviews medical and other records to evaluate quality and appropriateness of care; and agency policies and procedures to ensure they are consistent with federal and state mandates; interviews individuals, family members, and health care professionals). The surveyor will prepare State of Deficiency reports (SODs) and any associated survey correspondence in a clear and concise manner. The incumbent will also ensure that Plans of Correction (POCs) comply with all applicable regulations and requirements. The surveyor will properly use the automated system to document survey activities and ensure that information is accurately and clearly presented. The surveyor will also review policy and procedure manuals for new agencies, review and monitor Home Health Aide Training Programs, and respond to public health emergencies and participate in preparedness related activities, training and exercises as directed and needed.

Additional Comments Frequent travel, including overnights and travel to areas that may not be served by public transportation, is required.

This position requires travel including to locations that may not be serviced by public transportation. Posted Salary range includes the statutory hiring rate at step 4 and job rate for PS&T Grade 20 ($81,720 - $91,381) and an additional Geographic Pay Differential of $35,000.
