Job Description

Total Parenteral Nutrition (TPN)
1. Review medication profile for potential drug-nutrient interactions and/or incompatibilities and communicate with dieticians, physicians and nurses when clarification is required.
2. Check electrolyte levels when required.
3. Create 'invents' (interventions_ for all questionable orders.
4. On a daily basis, enter all orders in the computer before 12 noon to be accessed electronically by Central Admixture Pharmacy Services (CAPS) on a timely manner.
5. Upon receiving the prepared solutions from the vendor, check the labels of the TPN/PPN solution against the original order for accuracy prior to delivery/
6. Ensure proper storage and delivery of solutions.
7. Fill prescription in individual folder and file log sheet in the log book.
8. Adhere to scheduled lunch and breaks as determined by the supervisor to ensure proper coverage in the area.

B. Chemotherapy Products
1. Review and verify chemotherapy orders for inpatients an outpatients on a timely manner in order to prepare without delay.
2. Prepare chemotherapy preparation in compliance with all applicable health system policy and procedures.
3. Properly handle chemotherapy agents using safety precautions according to set guidelines (gloves, gown, mask, proper use of laminar flow hood, etc.)
4. Ensure that all chemotherapy preparations are delivered to the appropriate area on a timely basis.
5. Communicate medication order clarification with other pharmacy staff.
6. Perform other duties as assigned by supervisors.
7. Escalate inappropriate orders to appropriate providers.

C. Other IV Products
1. Prepare large and small parenteral solutions where there are no commercially available products.
2. Responsible for antimicrobial and other agents dosed according to patient's weight.
3. Compound oral suspension or solution for pediatric or geriatric patients who cannot swallow whole tablets.

D. Distributive/Clinical Functions:
1. Report any adverse drug reactions (ADRs).
2. Provide educational lectures to pharmacy and nursing staff as needed.
3. Supervise and direct support personnel (pharmacy technicians, students, etc.)
4. Verify the daily activities of pharmacy technicians.
5. Verify the accuracy of completed chemo preparations taking no coordinate release of order to outside vendor when such service line is in place.

Minimum Qualifications
1. Possession of a valid NY State license to practice pharmacy.
2. Two years of post licensure experience; or
3. Completion of a post graduate specialty residency or fellowship program; or
4. Current Pharmacy Board Certification.
