Job Description

Under supervision, plans, designs and evaluates programs and systems (or subdivisions thereof) relating to health care services, and implements these by appropriate analytical, liaison, consultative and research functions.

The primary goal of maternal health quality care coordinators will be to improve the population health of high-risk obstetrical patients and reduce the incidence of severe maternal morbidity (SMM) and maternal mortality (MM). The High Risk Maternal Medical Home (HRMMH) is aimed at improving the quality of high-risk maternity care, improving maternal and infant outcomes and reducing SMM and MM.

Maternal Care Coordinators/Healthcare Program, Planner/Analyst will provide information to patients on appropriate use of Obstetrical ED visits, connect them to local resources in their communities (such as doula services), information specific to their MMH provider, navigating the H+H system, referrals to public charge, behavioral health support, etc.

General tasks and responsibilities will include:
  • Prepares program guidelines, designs or protocols, and delineates the problems relating to their implementation.
  • Plans and conducts studies such as:
    • Feasibility Studies and Resource Analysis;
    • Content and Standards for the professional components of health care projects;
    • Analyst of socio-economic and other demographic data;
    • Public policy and Legal Restraints; and,
    • Staffing patterns.
  • Defines and develops data needed to provide the factual basis for plans. Converts data into quantification measures for utilization Ratios and Patterns, Manpower and Staffing needs, Pricing and Costs Formula and other studies.
  • Maintains liaison and coordinates activities with operational health facilities personnel, representatives of the community and involved governmental or health care agencies.
  • Consults with personnel in other segments of the Corporation involved in the financial, operational or professional components of health care program plans.
  • Participates in providing technical/professional assistance to operational facilities by assisting in development of training materials, preparation of interpretive bulletins and procedures, etc.
  • Participates in developing strategies and alternative approaches for improvement of specified health care programs.
  • Participates in designing evaluative measures and procedures by which program efficiency and cost effectiveness can be audited and participates in the evaluation audits.
  • Performs other related tasks
  • Assisting with patient outreach, care coordination services, and navigation of the health care system.
    • Utilizing population management data and resources to identify at risk patients and facilitate appropriate support and resources.
    • Providing patient outreach, implementing appropriate evidence based guidelines, monitoring care plan compliance and removing barriers to success.
    • Facilitating patient engagement with their medical home, assisting with access to and navigation of the health care system.
    • Documenting interventions in the patient's electronic medical record.
    • Participating in care team meetings, performance improvement and patient satisfaction initiatives.
    • Reporting issues and system barriers to efficient and effective care management.
    • Maintaining compliance with organizational policies, procedures and evidence based guidelines.
    • Maintaining and inputting data into MMH database to further inform care improvements and management of the MMH program.


Minimum Qualifications
1. A Baccalaureate Degree from an accredited college or university in Business Administration, Engineering, Social Sciences, Health Care Specialization, Physical Sciences or related programs; and,
A minimum of two years responsible experience directly related to health care program planning, research, design, operation, evaluation and analysis; or,
2. A Masters Degree from an accredited college or university in Public Health, Public or Hospital Administration, Health Care Specialization, Business Administration or related disciplines; and,
A minimum of one year experience directly related to health care program planning, research, design, operation, evaluation and analysis; or,
3. A satisfactory equivalent combination of training, education and experience; and,
4. Demonstrated skills in written or oral communication.
