The most challenging part of this role? Effectively working with multiple project teams and stakeholders on project implementations. Our teams require flexibility utilizing various methodologies and tools. You'll need to drive collaboration in a cross-matrix environment leveraging your ability to build relationships and keep on track with the project scope and timeline. The Population Health Lead is responsible for the coordination and completion of projects across various functional areas. Typically, employees in this function are working towards or have achieved certification in Project Management.

Primary Responsibilities:

  • Manage projects from inception to completion through QI science methodology documentation including: Project charters, PDSA cycles, Key Driver Diagrams, GEMBAs, Run Charts and Paretos
  • Complete data analysis for projects to determine progress and if SMART aims are achieved or root causes of performance
  • Lead multidisciplinary teams to complete project tasks
  • Recommend appropriate resolutions
  • Define performance and results and assist management with vendors and service providers
  • Able to define direction of complex work for self and others with minimal oversight and direction
  • Proficient at all consultant level requirements plus:
    • Self-directed, independent and track record of initiation and coordination for complex topics
    • Apply and coach others to apply QI science methodology and tools (e.g., IHI model, PDSA) to drive quality across multiple populations
    • Comfortable working through ambiguity toward clarity
  • Define performance and results and management of whole populations to include resources which are external to United (ie, providers, other MCPs, the Ohio Department of Medicaid and third-party vendors)
  • Provide direction and performance feedback to the teams engaged with population health work
  • Open to innovation, creative ideas, will completely re-engineer work processes when needed
  • Coaches, provides feedback, and guides others. Acts as a resource for others with less experience
  • Demonstrated ability to lead, influence change, initiate and shape work with colleagues inside and outside the organization to achieve goals
  • Use data analytics, assist with the identification of opportunities for improvement at multiple levels
  • Provide subject matter expertise in areas including project scope definition, risk identification, project methodology, resource allocation, facilitation, and other areas of expertise based on established project management techniques
  • Make presentations to management on project updates, project cycle, and expected results
  • Ensure projects are completed on time and in scope

Required Qualifications:

  • 2+ years of project management experience using data and information to make business decisions
  • Experience in Healthcare Quality, Healthcare Analytics, or Quality Improvement
  • Experience with IHI methodology or Quality Improvement Process
  • Experience analyzing data sets
  • Proficiency with Microsoft Excel and Microsoft PowerPoint with the ability to edit, create, save and send spreadsheets and documents
  • Resident of the state of Ohio

Preferred Qualifications:

  • PMP Certification
  • CPHQ Certification
  • Lean Six Sigma
