Assist in assessing the activities interests and needs of residents.
Participating in planning and implementing of group and 1:1 activity programs for residents based on their known interests and needs.
Encourage residents to participate in activities; transport residents to and from activities.
Facilitate group and 1:1 activities; observe participation and get resident feedback about programs.
Provide support to MDS assessment process, including the completion of surveys and assessment documentation.
Assist with department administrative functions including maintenance of supplies and materials, program equipment, production/posting of the monthly calendar, etc.
Other duties as assigned
Qualifications include:
Experience in leading group and individual activities within a long term care facility, adult day program or similar environment.
Excellent interpersonal and organization skills as well as the focus and work ethic to complete assignments on time, independently and/or as part of the team.
Proficiency using PointClickCare or other Healthcare Information system as well as Microsoft Office applications such as PowerPoint and Publisher.
Recreation Therapist Certification is a plus as is experience as a Certified Nursing Assistant.
