The Addiction Counselor serves as an integral member of the multidisciplinary team and participates in and/or leads the development and implementation of clinically sound, culturally relevant, and medically monitored treatment programs, case management, care coordination, and other supportive and consultative services for patients in Substance Use Disorder (SUD) treatment, in a sensitive and caring manner, and promotes the notion of recovery and self-sufficiency, as a goal of treatment. Ensures optimal utilization of resources and services and compliance with all applicable local, state, and federal rules and regulations and NYC Health+ Hospitals standards of care to improve outcomes and the patient experience,

Under supervision, participates in and/or performs functions related to counseling, diagnostics, treatment, treatment planning, rehabilitation, assessment, intervention, and other supportive services for patients in SUD treatment.

General Functional Job Description:
  • Screening and assessing patients in need of addiction or detoxification treatment.
  • Developing appropriate evaluation reports and /or psycho-social/addiction histories for new patients.
  • Making decisions regarding frequency and intensity of treatment during intake and assessment process.
  • Interviewing and providing counseling to patients admitted to addiction treatment program or detoxification unit.
  • Providing individual and group theme-centered therapeutic services and conducting educational groups dealing with the disease of addiction and transition of recovering persons, etc.,
  • Providing family therapeutic counseling services and leading groups dealing with alcohol, drug, and chemical dependency education, co-dependency, dynamics of dependency on the family structure and children of substance abusing parents, etc.
  • Negotiating and providing direction to patients regarding contingency contracting, (appointment scheduling, screening mechanisms, etc.)
  • Providing home visits, follow-up, and aftercare, as necessary
  • Identifying and isolating specific problems of patients, assisting in obtaining entitlements.
  • Providing clinical and treatment direction to subordinate staff.
  • Participating in interdisciplinary training, staff development, and Grand Rounds activities and acting as preceptor for students, interns, and those meeting field placement requirements.
  • Reviewing and co-signing clinical documentation, including chart notes, treatment/service plans and updates, and discharge summaries.
  • Participating in development of treatment and discharge plans.
    • Conducting supportive groups and community meetings.
  • Upon discharge, referring patients for rehabilitation programs and other treatment modalities.
  • Working closely with related social and community agencies.
  • Other tasks that may be assigned.
  • Providing expert addiction consult for any patient receiving treatment in the Medical ED; substance use screening, brief intervention, motivational interviewing, overdose prevention education and teaching, and linkage to follow-up care.
  • Be a part of an exciting team of social workers, addiction counselors, and peer counselors; collaborating with medical providers to provide a crucial service created to combat the opioid epidemic and as an innovative approach to addiction treatment.
  • Schedule: Rotating or fixed schedule to cover evening hours, overnights, weekends, and holidays.


Minimum Qualifications

Any individual working in a NYS Office of Alcoholism and Substance Abuse Services (OASAS)-Certified Treatment
Program or newly-hired into the System in the title of Addiction Counselor in a NYC Health + Hospitals health care
setting must have proof of completion of OASAS "Supporting Recovery with Medications for Addiction Treatment
(MAT)" training.
For Assignment to Level I
1. Valid Credentialed Alcoholism and Substance Abuse Counselor (CASAC) - Level 1 Certificate issued by the
NYS Office of Alcoholism and Substance Abuse Services (OASAS); or
2. Is otherwise considered a Qualified Health Professional (QHP) based on licensure, certification and/or
registration along with appropriate experience and/or training as defined in Section 800.3 (l) of the OASAS
Treatment Services: General Provisions, Title 14 NYCRR Part 800
