Job Description

Position Summary:
The Addiction Counselor is an integral member of the treatment team within the Substance Abuse Services. Along with other members of the team, the Addiction Counselor
serves as primary counselor to a caseload of patients and participates in the implementation of a methadone maintenance and drug-free program by engaging in diagnostic,
treatment and consultation activities. Under general supervision, plans, directs, coordinates and monitors a program of medically/psychiatrically approved addiction counseling for adults.

Essential Job Functions:
• Utilize the LOCADTR, of Department of Health Commerce System, to determine the level
care for patients entering the Methadone treatment program.
• May act as a Liaison between Senior staff and entry level.
• Act as an integral member of the Services and participates in various staff meetings and various types of in-service training.
• Provides in-service training to Junior Staff within area(s) of expertise as needed.
• Participates in developing treatment plans and discharging plans based on assessment with the multi-disciplinary treatment team.
• Utilize EPIC, Myam, New York City HR Stars and Health Commerce Locdtr
• Explores new developments within the fields of Addiction Counseling and brings information to the Department of Behavioral Health and to the Substance Abuse Services. applies new techniques and skills in a continuing effort to provide effective treatment and rehabilitation modalities.
• Conducts individual counseling of a supportive nature, the goal of which is to motivate patients for treatment, engage in treatment, guide patient toward an understanding of their relapse triggers, provide mechanisms to overcome urges and triggers, and help work through problems that act as barriers to the patient's total rehabilitation.
• Provides education regarding substance abuse, recovery/relapse prevention, HIV, HEP A,B,C courses of Illness

• Conducts an initial bio-psychosocial which includes assessment of patient's assets and limitations; identifies psychosocial, personal and social problems; evaluates motivation/readiness
for treatment and rehabilitation; gathers information regarding psychosocial medical and
• ,psychiatric history.
• Develops treatment plans and discharges plan based on assessment with the multi-disciplinary treatment team.
• Performs intake interviews and screens patients for admission. Assess and refers patients to other facilities/treatment who do not meet the admission criteria.
• Completes medical record requirements that include a bio-psychosocial assessment, formulation and completion of a comprehensive treatment phm within 30 days and summaries (3 and 6 months) for patients less than a year. Every 6 months thereafter.
• Completes progress notes in an appropriate and timely manner based on patient's attendance frequency indicated. in the treatment plan.
• Completes letters, forms, and applications as needed to facilitate patient in obtaining services.
• Asses issues regarding histories of violence, elderly and child abuse, sexual abuse and trauma.
• Ensures confidentiality of case records.

Minimum Qualifications

Any individual working in a NYS Office of Alcoholism and Substance Abuse Services (OASAS)-Certified Treatment
Program or newly-hired into the System in the title of Addiction Counselor in a NYC Health + Hospitals health care
setting must have proof of completion of OASAS "Supporting Recovery with Medications for Addiction Treatment
(MAT)" training.

1. Valid Credentialed Alcoholism and Substance Abuse Counselor (CASAC) - Level 1 Certificate issued by the
NYS Office of Alcoholism and Substance Abuse Services (OASAS); or
2. Is otherwise considered a Qualified Health Professional (QHP) based on licensure, certification and/or
registration along with appropriate experience and/or training as defined in Section 800.3 (l) of the OASAS
Treatment Services: General Provisions, Title 14 NYCRR Part 800
