Washington County Behavioral Health Division within the Department of Health and Human Services is recruiting for 2 openings, an Adult Intensive Care Coordinator to join the Intensive Care Coordination (ICC) team and a Forensic Exceptional Needs Care Coordinator which provides care coordination to individuals under the Aid & Assist Program who are determined as meeting Choice Program criteria as per contract with the Oregon Health Authority.

About the Adult Intensive Care Coordinator
This position will primarily serve adults with complex behavioral health needs, with the primary goal of improving health outcomes through collaboration with the individual, system providers and partners, and other organizations providing supports. In this role, you will work to ensure Oregon Health Plan members residing in Washington County have access to care coordination regardless of eligibility for specific services. You will work with adult clients to provide needs assessment, care planning, connection to service delivery systems, and coordination with behavioral health, physical health, and dental providers. Using a person-centered approach, you will facilitate interdisciplinary team meetings and coordinate with partners including legal systems, Oregon Department of Human Services, Aging and People with Disabilities (APD), and Developmental Disability services with the goal of improving communication and health outcomes in all areas.

About the Forensic Exceptional Needs Care Coordinator:
In this role, you will primarily serve adults with complex behavioral health needs under the Aid and Assist program who are meeting Choice Program eligibility with the primary goal of collaboration with the individual, system providers/partners, and other organizations providing supports. This role will also be conducting Civil Commitment investigations and Trial Visit Monitoring and will be a bridge between the Choice and Civil Commitment Programs. Primary duties may include, but are not limited to:

  1. Meeting regularly with individuals open to Choice Program to coordinate care and develop plans to determine placement needs and support that may aid in the success of placement at the lowest level of care possible.
  2. Serving as an informational resource for behavioral health providers and other community partners for assigned clients; specifically, regarding the history of needs in care and approaches that have proven successful.
  3. Serving as an informational resource for involved family members and assigned guardians.
  4. Participating in staff meetings in person or by phone for individuals assigned.
  5. Developing knowledge of the service continuum available to specific clients open to Choice Program in Washington County.
  6. Identifying specific services and supports that would be of a benefit to assigned clients.
  7. Providing Pre-Commitment services to those diagnosed with serious and persistent mental illness (SPMI) for the purpose of determining the need for involuntary hospital care.
  8. Assessing individuals and determining if specific legal criteria are met for civil commitment. Based on the timelines set in Oregon Statutes, preparing thorough reports that are professional and support the determination made regarding commitment recommendations.
  9. Providing Trial Visit monitoring for individuals who have been civilly committed by the court and are completing commitment in the community.
  10. Traveling to various settings to meet with individuals and staff, including acute care facilities.
  11. Maintaining positive, professional relationships with agency providers, system partners and hospitals serving this client population.
  12. Providing coverage for co-workers during their absence to assure services are not disrupted due to such absence.
  13. Documenting all services provided as described above and ensuring that documentation is current in the event someone would need to cover for an absence.

ESSENTIAL JOB DUTIES

MINIMUM QUALIFICATIONS

Education and Experience:
  • A master's degree in a clinical field related to the diagnosis, etiology and treatment of mental health disorders; AND three (3) years of increasingly responsible mental health experience in assisting individuals with severe and persistent mental illness in accessing needed services and support; AND
  • Must be certified by the Mental Health and Addictions Certification Board of Oregon as a Qualified Mental Health Professional or hold an approved clinical license and meet the criteria of a Qualified Mental Health Professional as defined in the Oregon Administrative Rules.

Additional Requirement (Only for Forensic Exceptional Needs Care Coordinator):
  • This position requires to receive the state certification as a Civil Commitment Investigator/Examiner within 60 days upon hired.
