Overview

The Afterhours Care Coordinator assumes accountability for the coordination and evaluation of patient care during afterhours. This position is responsible for afterhours communication; making patient care decisions and coordinating and assigning care through Homecare providers and accepting reports from and providing reports to care centers regarding afterhours activities.

Responsibilities

  • Documents incoming communication (calls, faxes, texts, email) and coordination of services on approved reporting form(s).
  • Makes patient care decisions in response to incoming communication, using current EMR and professional nursing judgment. Such responses may include giving advice or direction to the patient/caregiver, contacting the Physician or scheduling a visit.
  • Provides guidance and direction to home care clinical staff regarding patient care and clinical issues, symptom management, patient incidents and/or injuries.
  • Coordinates and assigns visits to home care which he/she identifies as medically necessary or requested by patients/caregiver to meet patients' needs.
  • Monitors high risk patient report, ensures high risk patient calls are completed and visits are scheduled, and reports all after-hour high risk patient calls to care center leadership.
  • Regularly communicates with clerical staff to coordinate patient care and works in cooperation with appropriate services.
  • Oversees after-hour, weekend and holiday schedules, as prepared by branch office. Makes adjustments to schedule in response to patient and/or staffing changes.
  • Responds to patient care complaints in a courteous, professional and appropriate manner.
  • Performs the duties and responsibilities of the Intake Coordinator position, as necessary. In addition, may assign admission visit to appropriate staff when weekend/holiday admission is indicated.
  • Initiates/coordinates with the physician and obtains orders for admissions and schedule the visit with the nurse the same day.
  • Performs other duties as assigned.

Qualifications

Required
  • Current and unencumbered license to practice as a Registered Nurse. An employee in this role is only able to provide clinical guidance to patients in states in which the employee possesses a current, unencumbered license. May possess either a single state license in each non-compact state and/or a compact license in their state of residence that is accepted in all Nurse License Compact (NLC) participating states.
    • Nurse Licensure Compact (NLC) documents the following: The mutual recognition model of nurse licensure allows a nurse to have one license (in his or her state of residency) and to practice in other states (both physical and electronic), subject to each state's practice law and regulation. Nurses must legally reside in a NLC state to be eligible to have a multi-state license.
  • Three (3) years' experience as a Registered Nurse. One of those years must consist of full-time experience in providing direct patient care in the home health/hospice setting.
  • Current CPR certification.
  • Demonstrates knowledge of Federal/State rules and regulations, ACHC standards, and other regulatory requirements.
  • Demonstrates strong fiscal planning and management skills.

Preferred
  • Two (2) years' home health/hospice experience.
