Job Description

Housing for Health (HfH) at NYC Health + Hospitals (H+H) recognizes that the chronic health issues of unhoused individuals cannot be treated without stable housing. This initiative seeks to improve the health and wellbeing of New Yorkers by focusing on four strategic areas: housing navigation services, medical respite beds, affordable housing on hospital property, and social service support for patients in permanent housing.

The Housing team works closely with H+H care teams to identify patients; accesses suitable opportunities through strategic partnerships with government agencies, housing developers and nonprofit organizations; and supports patients through their progression to permanent housing. The team reports to the Assistant Vice President of Housing and Real Estate and sits within the Office of Managed Care.

The Assistant Director of Medical Respite Operations will work under the Senior Director to lead program and operations for NYC Health + Hospitals flagship medical respite program. HfH's Medical Respite program provides interim housing and services for persons experiencing homelessness who have clinical needs that do not require acute hospital level of care but cannot be adequately served in a NYC Department of Homeless Services (DHS) shelter or on the street. Our current Medical Respite Program up to 90 days of interim housing with 24/7 staffing that allows patients with clinical needs to receive services including: on-site clinical services (i.e. wound care, PT/OT, IV infusion), care coordination and medication monitoring, and intensive housing case management with the goal of permanent housing post respite stay.

General tasks and responsibilities will include:
  • Oversee medical respite referral and bed management procedures, in coordination with respite operators and H+H facilities. Review escalations from respite PMs when needed
  • Oversee care coordination efforts with H+H care teams, including acute care facilities, Primary Care Safety Net clinics, and other longitudinal care teams, including development of systems to improve coordination and connections to care
  • Manage and provide supervision of respite project managers (PMs), prioritize organizational needs, and delegate tasks appropriately
  • Oversee development of respite policies and procedures, training, and operational guides that provide comprehensive overview of expectations of respite operators. Support respite operators to adopt policies and procedures, delegating to respite PMs when appropriate
  • Oversee respite inter-agency policy and operations, including with NYC Department of Homeless Services' Reasonable Accommodations (RAs) process for patients discharging to shelter from respite and/or mutual clients Collaborate with Medical Director on system management of projects including planning, defining scope, design, operations, execution and reporting.
  • Oversee expansion of medical respite program, including procurement of new contracts, identifying partners' and HfH needs, and developing new innovative systems to adapt to program's growth
  • Identify gaps and other opportunities for systems improvement and development


Epic Build, Data Management, and Quality Improvement
  • Develop, maintain, and oversee medical respite Epic build dashboards, including support of Respite PM in data management and on-going respite reporting
  • Collaborate with Epic and HfH clinical teams to optimize the Epic Care Link Respite build, identifying opportunities and tools to improve workflows, documentation and performance management
  • Oversee ongoing Epic training to respite operators, monitoring and evaluation of Epic documentation, and quality improvement efforts
  • Collaborate with HfH clinical and housing teams to identify gaps in care and areas for program improvement, leading quality improvement efforts
  • Collaborate with various teams to submit articles to peer reviewed journals Oversee other respite data management systems, as identified


Other HfH Programmatic Support
  • Oversee financial management of all Housing for Health program budget cross cutting multiple programs and sources including grants, philanthropy and H+H expense
  • Collaborate with HfH team for program evaluation, including research opportunities and external reports on HfH projects
  • Manage and assist with special projects as requested by Assistant Vice President

Minimum Qualifications

* Qualification for Central Office.

# Qualification for Health Care Facilities.
  1. A Baccalaureate Degree from an accredited college or university in Business Administration, Public Administration, Hospital Administration, Social Sciences, Management, Health Care Administration or an approved related field in an accredited college or university; and,
  2. Six* or five# years of progressively responsible planning, design and program development experience pertaining to the operation of a major segment or all of a hospital or health care facility including substantial exposure in meeting community health needs, health care planning, design and program implementation functions with at least three* or two# years of responsible level administrative experience coordination health care planning, design, development and public policy programs; or,
  3. A Master's degree from an accredited college or university in Social Science, Hospital Administration, Public Administration, Business Administration, Health Care Administration, Administrative Medicine in Public Health or approved related field; and,
  4. Five* or four# years of progressively responsible experience as described in (2) with at least two years of responsible managerial and highly responsible level administrative experience coordinating health care planning, design, development or public policy programs; or,
  5. A satisfactory equivalent combination of education, training and experience.


Department Preferences

Preference will be given to qualified candidates with the following knowledge, abilities, education, experience and/or skills:

EDUCATIONAL LEVEL:
  • Graduate degree in public administration, urban planning, public health, or similar discipline OR
  • Undergraduate degree with at least three years' experience with project management and hands on data analysis or
  • A satisfactory equivalent of education, training and/or experience.

PREFERRED SKILLS:
  • New York City residency (Brooklyn, Bronx, Manhattan, Queens or Staten Island) preferred.
  • Professional and/or direct experience in housing, homelessness, urban planning, or health care administration.
  • Experience with SAS, SQL, R and/or Tableau
  • Demonstrates initiative, sound judgment and flexibility; possess effective time management and organizational skills; and is able to work independently within a team.
  • Excellent verbal and written communication skills as well as quantitative skills.
  • Detailed oriented and a commitment to innovation
  • Strong interpersonal, communication and group collaboration skills are essential.

COMPUTER PROGRAMS/SOFTWARE OPERATED:
  • Advanced skills in Microsoft Office applications including exceptional Excel, PowerPoint, and Word and project management tools.
  • Experience with SAS, SQL, R and/or Tableau
