Job Description

  • Develops opportunities for new and enhanced referral relationships with, but not limited to hospitals, rehabilitation centers, commercial payers, patients' families and the community.
  • Maintains an on-site presence at the geographically assigned acute hospitals; establish relations with hospital leaders, case managers, discharge planners and social-workers.
  • Conduct chart reviews of potential admissions when referred
  • Provide and coordinate with Gouverneur CEO/CMO presentations / in-services on focused clinical disciplines: Discharge Planners / Case Managers, Residents, Attending Physicians
  • Meet with patients and families to explain Gouverneur services and benefits over other Post-Acute sites
  • Collaborates with facility leadership and Marketing team to develop promotional materials and plan events to educate referral sources on NYC H+H PAC Services
  • Ensure that all required data for referrals is entered into Allscripts and coordinate with Admissions team to ensure that timely decision on admission is made and status is communicated.
  • Facilitate discharge process by following-up with admissions team and hospital representative on status of financial clearance.
  • Offer and arrange facility tours


Minimum Qualifications

1. Licensed and currently registered to practice as a professional nurse in New York State, plus (2) or (3), below.
2. Master's Degree in Nursing Administration, Education, Clinical Specialty or equivalent field and five years of satisfactory nursing experience, of which three years shall have been in a supervisory or teaching capacity; or a Baccalaureate Degree in Nursing and six years of satisfactory nursing experience, four years of which shall have been in a supervisory or teaching capacity.
3. A satisfactory equivalent of education and experience. However, all candidates must be licensed and currently registered to practice as a Professional Nurse in New York State.

Additional Qualifications

  • Must be customer service orientated and demonstrate strong interpersonal, verbal and written communication skills.
  • Demonstrated ability to take initiative and exercise good judgement with staff, patients, system stakeholders and other healthcare/community partners.
  • Prior experience working in the healthcare industry, preferably in Post-Acute or Long-Term Care.
  • Patient Review Instrument (PRI) Certified
  • Knowledge of PDPM, RUGS III, Medicare and Medicaid eligibilities
  • Managed Care eligibility requirements
  • Must have excellent computer skills and knowledge of databases.
  • Experience working with electronic health records and familiarity with medical terminology and coding.
  • Work hours will primarily will be M-F, must be available to work weekends if required.
  • Must be able to travel to work locations within NYC.
