Job Description

PURPOSE OF POSITION:
  • Works in conjunction with Gotham Health Nursing, Administrative, and Medical leadership to develop and implement the strategic plan of the assigned community health sites. Acts as a change agent, leader and role model for nursing, with the goal of aligning nursing practice with regulatory and accrediting agencies' standards in achieving and sustaining optimal patient outcomes. . Provides guidance and develops action plans to address patient care issues.


AREAS OF RESPONSIBILITIES:
• Finance and Budget: Assists site leadership in developing operating budget for personnel service (PS) and other than personnel services (OTPS) . Monitors clinical and financial variance vs target metrics/activity on a monthly basis. Consistently reviews department budget and implements cost containment strategies as needed. As part of the site leadership team, is responsible for the designated sites nursing clinical metrics including T2T as well as care gaps.
• Patient Relations: Works with Patient Relationship representatives in the identification of patient complaint trends and develops collaborative plans of correction. Works with departmental leadership to develop strategies to enhance the patient experience.

DUTIES AND RESPONSIBILITIES:
• Ensures the development, implementation, and enforcement of all nursing policies and procedures in the primary care setting.
• Participates in the quality assurance program for patient care, process improvement, and staff engagement/performance. Develops and directs continuous quality improvement programs which achieve targeted patient quality of care outcomes
• Complies with applicable laws and Collaborates with Nursing Education to ensure of nursing staff orientation, education, and ongoing staff training in accordance with state regulations and organizational requirements
• Works collaboratively with Infection control to ensure effective OSHA/Infection Control programs. Ensures all clinical personnel practice infection control and standard precautions maintain compliance with all safety regulations.
• Identifies work related improvement opportunities and analyzes them to identify causes/find solutions that improve teamwork and patient satisfaction.
• Expeditiously and appropriately addresses concerns entered into VOICE system and collaborates with Risk Management for problem-solving and resolution.
• Acts decisively to implement solutions to resolve critical issues or improve processes.
• Communicates clear understanding of job responsibilities, task objectives, deadlines and performance expectations.
• Shares up-to-date information on nursing activities and policy changes in a timely manner.
• Listens attentively to others. Tries to understand the meaning of the communication of others, perceives their needs and interests.
• Works in harmony with others as a team leader.
• Participates in on site regulatory surveys, i.e. VFC, Laboratory, Article 28, HRSA and development of Corrective Action Plans as needed.

• Oversees the maintenance of all clinical equipment.
• Oversees site Infection Prevention and Control Program.
• Site Nursing lead for creative and innovative paths forward in the front lines of nursing and patient care.
• Responsible for care gap metrics at site level
• Must possess an understanding of Gotham's Health overall strategic goals and have demonstrate a sense of commitment and passion to achieving those Goals.
• Monitor pre-visit planning at sites to include daily huddles.
• Travel to sites in areas of responsibility.
• Valid Driver's license

Minimum Qualifications
1. Licensed and currently registered to practice as a Professional Nurse in New York State, plus (2) or (3) below.
2. A Master's Degree in Nursing Administration, Education or an equivalent field and a minimum of six years of satisfactory nursing experience, of which four years shall have been in a supervisory, teaching or administrative capacity; or,
3. A Baccalaureate Degree in Nursing and seven years of satisfactory nursing experience, five years of which shall have been in a supervisory or teaching capacity; or,
4. A satisfactory equivalent combination of education and experience. However, all candidates must be licensed and currently registered to practice as a Professional Nurse in New York State.

Department Preferences

QUALIFICATIONS FOR THE JOB:

Active, unrestricted license as a Registered Professional Nurse in New York State

Excellent communication skills, both oral and written.

CERTIFICATION(S)/LICENSE(S):

Basic Cardiac Life Support - required

Current certification in Ambulatory Care Nursing, preferred.

EDUCATIONAL LEVEL:

Master's Degree in Nursing or related field required.

KNOWLEDGEABLE IN:

EPIC electronic health record
