The role you'll contribute:

The Nurse Navigator manages and coordinates aspects of care for seamless transition of care for identified patients. This is accomplished through multidisciplinary interaction, nursing management, education, care coordination, resource allocation, and communication. The Nurse Navigator is responsible for ensuring a care plan is carried out in partnership with the persons at the center of the care plan. He/she will work as part of the multidisciplinary team to implement measures targeted to ensure seamless transitions. The goal will be achieved through measures implemented that:

• Work with members of the healthcare team to facilitate patient healthcare

• Promote timely access to care

• Increase utilization of preventative care

• Build awareness among the healthcare team about the importance of patient navigation

The value you'll bring to the team:

• Serve as contact person, advocate, resource for care team, patient and caregivers

• Help ensure patient and family understand processes, care strategies, and recommend actions and provide information in coordination with healthcare providers

• Assist with implementing plan of care to achieve treatment goals

• Recommend additions to or modifications of referring orders

• Collaborate with multi-disciplinary team to coordinate post-acute needs, including transition to outpatient setting

• Establish and promote a collaborative relationship with physicians and other members of healthcare team

Qualifications
The expertise and experiences you'll need to succeed :

Minimum qualifications :

• Graduate of approved school of nursing

• Minimum of five years' nursing experience

• Texas RN licensure

• BLS
