Job Summary:

The Advanced Practice Provider will serve in a full-time role to provide quality care to patients at the SUNY Upstate Cancer Center, Downtown. This APP role will work in partnership with the Hematologist providing advance practice care to patients, families and the community through assessment, planning, prescribing and evaluation interventions.

Responsibilities Include:

Managing and seeing patients for their follow-up visits.

Initiating and completing treatment plans, continuing chemotherapy treatments.

Managing the in-basket and ensuring quality care of this patient population.

Responsible for prescription refills, in-basket management, disability paperwork, treatment plan signing and infusion coverage.

The APP is a member of the multidisciplinary healthcare team who demonstrates professionalism and leadership within the institution and exemplifies the Upstate mission, vision and values and acts in accordance with Upstate policies and procedures.

Minimum Qualifications:

Master's Degree as a Nurse Practitioner withRegistered Nurse First Assist (RNFA) or Physician Assistant or eligible.

NYS licensed/registered NP or PA required at time of appointment.

National Certification consistent to the clinical practice also required at time of appointment.

NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire.

CPR-BLS Certification required at the time of appointment.

Preferred Qualifications:

Prior experience in specialty area preferred.

Monday - Friday, Days
