Job Description:

SHIFT: 8 HR Days Monday-Friday from 0800-1700 and work weekends. If working a weekend, then will have a weekday off.

WEEKENDS: 2 weekend days a month

HOLIDAYS: 1 holiday on call

ON CALL: On call after 5 pm for unit that they are covering and weekend on call if scheduled to work. 

FLOATING: Per Ministry Policy

PATIENT RATIO: 22:1

PATIENT POPULATION: Could be mother baby, NICU, Med-Surg, IMCU, Telemetry, ICU, or Surgical patients.

GUARANTEE: 40-hrs

EMR: Meditech

DRESS CODE: Business casual dress code or grey scrubs.

REQUIRED CREDENTIALS: *Must be provided upon submission*

RN or LMSW in the state of employment is required.

LBSW accepted for associates with 5+ years of demonstrated success and experience in CHRISTUS Care Manager I role.

BLS preferred. Certification in Case Management preferred.

REQUIRED EXPERIENCE: 2 years; CM experience in acute care setting of 2 or more years, Meditech experience.

PREFERRED SKILLS: Midas experience.

2+ years of experience in case management processes in an acute care setting.

Knowledge and Experience with Electronic Health Records: Epic knowledge is beneficial.

Knowledge of CMS, Medicaid, commercial payer requirements, and financial & reimbursement processes.

Ability to review cases for the appropriate status of care, continued stay, and the utilization of discharge screens.

Must be proficient in word processing and the use of clinical software.

Conduct intake assessments with patients and families.

Ensuring that families feel sufficiently equipped and supported.

Developing individualized treatment plans that account for physical, financial, and other constraints.

Adjusting treatment protocols, as needed.

Suggesting and coordinating appropriate follow-up services to promote wellness.

Ensuring that patients have transport to and from their homes and all pertinent healthcare facilities.

Meeting with members of healthcare teams to discuss patients' progress, and to deliberate on appropriate care.

Checking to see whether patients have sufficient insurance coverage or finances to pay for their treatment.

Devising innovative solutions or suggesting appropriate referrals for patients who cannot afford to access treatment.
