DUTIES AND RESPONSIBILITIES :

  • Complete weekly GRID updates and Continued Stay Criteria Docs sent to Utilization Management.
  • Complete CONNECTS, treatment plans, and crisis planning .
  • Enter service orders upon admission of new residents .
  • Transport individuals to program-related appointments as needed. .
  • Assists individuals in applying for and obtaining vital records.
  • Assists individuals in applying for and obtaining food stamps.
  • Submits referrals to appropriate services (SOAR, Supported Employment, Case Management, Medical Appointments, etc.)
  • Assists individuals in applying for housing vouchers or permanent supportive housing when appropriate.
  • Document three face-to-face and/or over-the-phone sessions daily in ECR; including complaints, results of assessments, etc
  • Maintain accurate and detailed participant records
  • Completes all mandatory annual trainings
  • Establishes professional and ethical relationships with individuals, families, and coworkers. Including maintaining professional boundaries
  • Exhibits good communication skills (oral, written, and active listening)
  • Communicates suggestions and/or complaints through appropriate channels
  • Participates in weekly residential team meetings
  • Other responsibilities and tasks as assigned


Requirements

  • Bachelor's Degree in Helping Field
  • At least one year's experience working in mental health or substance use disorder field preferred.
  • Must be able to pass background investigation, drug screen and have good 7year motor vehicle report.
  • Bilingual abilities a plus
