Case Manager

Luxor Nursing and Rehabilitation at Mills Pond is seeking a Case Manager. Long-term care experience preferred. Candidates must be a current NY Licensed Practical Nurse (LPN).

The Case Manager Assists residents and the Nursing Facility's Interdisciplinary Team to achieve residents' goals of a successful rehab stay and safe discharge back to the community. Case Manager will be responsible to assess treatment needs; develop, monitor, and evaluate treatment plans and progress; facilitating interdisciplinary approaches; monitoring resident progress performance and act as the key clinical & social liaison with resident family and/or external parties.

Responsibilities include but not limited to:

• Collaborate with hospital to provide updates and progress on mutual patients
• Complete discharge from facility/ re-integration back to community
• Scheduling appointments post discharge
• Sending medication list to physician's post discharge to ensure continuity of care
• Meeting with families to explain rehab process and expectations
• Working with admissions, clinical, rehab & social services staff
• Attending care plan meetings
• Pre discharge education to residents
• Some charting required
• LPN certification required
