As a Case Manager, you will monitor and manage patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance. You will be part of a robust and caring multidisciplinary team caring for our patients from admission through transfer or discharge. This position includes ED Case Management with behavioral health placement assistance.
Qualifications
Required Skills:
• Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
• Must be able to work in a stressful environment and take appropriate action.
• Demonstrates proficiency in verbal and written communication skills.
• Excellent interpersonal skills and the ability to interact effectively and professionally with individuals at all levels; both internal and external.
Position Requirements:
Associate's degree in nursing required
Bachelor of Science in nursing preferred
Current State of MI RN license required
1 year of case management experience preferred
Basic Life Support (BLS) certification required
Annual influenza vaccination or declination required
Position Details:
0.9 FTE (full-time, 36 hours per week)
8 hour shifts
Every 3rd weekend
No holidays
