DESCRIPTION

Population Health Services Organization helps support UC San Diego Health's Primary Care practices and are part of the team based model of care. Our department supports patient outreach focusing in on areas from wellness to end of life across the life span. Our goal is to ensure timely care connections, help with health education and inform patients about our model of right care, right place, right time. Our mission is to support the Quadruple AIM; quality, cost, patient and team experience / wellbeing. This is an exciting opportunity for a nurse with demonstrated nursing education interest and process improvement knowledge to grow with us as we build out our programs helping all patients, especially vulnerable populations across all ages who need additional help to better manage their health and safety at home.Applies skills and experience as a professional nurse educator to ensure optimal clinician knowledge and skills through education and training in either general or specialized nursing education and/or training.. Leads implementation of new programs as it relates to student expansion. This position is on site Monday - Friday with students. During student/ university breaks this position will be able to work from home as appropriate.

Key Responsibilities:
  • Works with our Population Health Clinical Nurse Educator (CNE) 4 to build out training modules for student rotations within Population Health.
  • Oversees day to day students; initial tech set up, orientation, outreach and documentation.
  • Evaluates student nurse competencies and skills and targets education and training.
  • Plans and delivers evidence-based education and training program curriculum. Incorporates principles of adult learning in the planning, implementation, and evaluation of programs.
  • Takes part in complex projects to design tools to validate staff nurse competencies with CNE4. Ensures that competencies reflect organizational goals, such as providing patient-population specific care, leadership development, and sensitivity to patient cultural differences.
  • Delivers nursing staff orientation programs and education and training activities. Identifies target audience, utilizes a broad variety of media and formats, and provides a variety of core education and training programs.
  • Ensures that nursing education and training activities are compliant with all legal and certification requirements, as well as medical center and organizational standards of care.Responds to regulatory mandates for research and management of sentinel events.
  • Pursues professional development and continuing education and training opportunities to keep abreast of regulatory and procedural changes related to clinical education and training programs.
  • Supports consistent nursing student rotations by acting as a liaison and Population health rotation promoter. In collaboration with nursing schools, builds learning experience that is congruent with Population Health programs and needs.
  • Participates in committees, task forces, and projects that evaluate a variety of issues, including quality of care, patient safety, and licensure and accreditation requirements as directed.

MINIMUM QUALIFICATIONS
  • Bachelor's Degree in Nursing.
  • Registered Nurse (RN) license issued by the State of California.
  • Three (3+) or more years of relevant experience.
  • Recent ambulatory care RN experience with progressive expertise in patient care services.
  • Thorough knowledge of delivering clinical nursing education and training material for orientation and skills/competency learning and leadership development.
  • Strong interpersonal skills, with the ability to provide education, training, and resources, and to develop positive working relationships with nursing staff and leadership.
  • Detail oriented, with proven organizational skills to effectively manage time, prioritize tasks, and see projects through to completion on deadline.
  • Strong skills in written and verbal communications, with the ability to collaborate productively in a team environment with all levels of internal and external personnel and to convey complex clinical information in a clear, concise manner.
  • Thorough knowledge of a broad spectrum of professional nursing practices and issues.Thorough knowledge of methodology for conducting evidence-based research, data collection, analysis, and reporting.
  • Strong critical thinking and problem-solving skills to manage multiple levels of information and responsibilities, and to quickly assess moderately-complex problems to develop potential solutions.
PREFERRED QUALIFICATIONS
  • RN experience with vulnerable, high-risk patients.
  • Primary Care RN experience.
  • Demonstrated PDSA/A3 or other lean process improvement to support greater efficiency and quality of care.

  • Must be able to work various hours and locations based on business needs.

Pay Transparency Act

Annual Full Pay Range: $116,000 - $224,200 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent: $55.56 - $107.38

Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
