The role you'll contribute:
Prepares and organizes pharmacy medications by name and dosage, as listed on patient orders.

PRINCIPLE DUTIES AND JOB RESPONSIBILITIES :
Pharmacy Operations
Actively performs all pharmacy technician functions and maintains active proficiency in:
• Standard work parameters
• Operational programs
• Dispensing and compounding systems
• Hospital computer systems
• Medication ordering systems
Maintains operational expertise in and actively provides coverage of the technician services provided by
• Central Pharmacy
• IV Room
AND MAY REQUIRE (at least two of the following):
• Oncology Pharmacy Satellite
• Operating Room Satellite
• CVICU Pharmacy Satellite
Oversees appropriate utilization of medication automation, technology, and computer systems
• Accurately interprets electronic records
• Maintains expertise in Cerner processes
• Manages proper and complete patient profile record
Accurately oversees the management of medication inventories and the preparation and dispensing of patient - specific medication doses
• Manages medication dispensing activities
• Ensures accuracy of all medication products prepared for dispensing
• Oversees oral and injectable product compounding
• Prevents improper drug, dose, dosage form selection, expiration dating, and barcode scanning issues when checking products
Pharmacy Practice
Completes critical medication order scrutiny, drug profile review, and patient monitoring to identify, prevent, or mitigate medication-related problems
• Improper drug, dose, or dosage form selection
• Dose frequency or timing
• Subtherapeutic dosage
• Over dosage
• Adverse drug reactions or drug allergy
• Drug interactions
• Therapeutic duplication
Proficiently verifies/enters medication orders and proactively communicates with medical staff to resolve problem medication orders
• Incorrect medication orders
• Ambiguous medication orders
• Non-formulary medication orders
• Home medication use requests
• Inappropriate therapy
Provides patient - centered care services
• Provides accurate, safe and timely medication therapy
• Initiates and facilitates appropriate therapy based on age, disease, and drug considerations
• Promotes cost-effective drug therapy and actively participates in stewardship activities and restricted medication/surveillance programs
Initiates and facilitates appropriate drug dosing and manages medication-related pharmacist consults (as appropriate for assignments):
• Pharmacokinetic monitoring
• Renal dosing
• TPN processing
Facilitates medication safety practices
• Identifies, mitigates, and reports potential and actual adverse drug events, adverse drug reactions, and medication errors
• Documents pharmacist interventions and appropriately documents in the patient medical record
Clinical Pharmacy Services
Provides support to direct patient care programs
• Provides accurate, safe, timely, and appropriate medication therapy based on patient needs
• Places practice priority on the delivery of patient-centered care
• Identifies high-risk patients and implements interventions to improve quality and safety
• Makes appropriate evidence-based, patient-centered medication recommendations
• Provides effective medication education and counseling to patients and caregivers
• Proficiently maintains patient information and efficiently evaluates patients' health care needs
Supports practice in decentralized areas
• Collaborates with other pharmacy staff
• Collaborates with care providers and Pharmacists to optimize care and improve services
Teamwork and Inter-Professional Collaboration
Presents to assigned areas on time, ready to work and does not exhibit unscheduled absenteeism
• Adheres to lunch and break schedule
Assures continuity of care between shifts, among pharmacy staff assignments, and across pharmacy practice areas
• Assures appropriate handoff between shifts and participates in Energizers/Meetings
• Accepts any necessary work from preceding and does not leave excessive work for following shift
• Reviews and catches up the rejected order queue as well as medication-related products to be checked (patient-specific doses, Pyxis fills, med batches, trays, etc.) prior to departing from shift
Supports other pharmacy operations and areas outside of current assignment
• Collaborates regarding medication order verification and medication availability
• Communicates with appropriate pharmacist regarding problem orders or patient care requests
• Provides coverage of medication order verification when areas are uncovered, or team needs support
Works effectively with all members of the healthcare team to ensure the best outcome for the patient
• Demonstrates sensitivity to the perspective of the patient, caregiver, or health care colleague in all communications
• Actively supports teammates (pharmacists, technicians, managers)
• Uses effective negotiation skills to resolve conflicts
Teaching, Training, Precepting, and Mentorship
Mentors new-staff and assists with training
Supports experiential training of pharmacy trainees in areas of expertise
• Actively engages in pharmacy technician student learning experiences
• Supports pharmacy student (IPPE and APPE) learning experiences
• Supports pharmacy resident training experiences
Quality Improvement, Regulatory Compliance, and Accreditation
Supports and participates in medication - use process improvements led by colleagues and peers
Supports regulatory compliance initiatives and supports institutional accreditation processes
Professional Activities and Practice Advancements
Maintains a commitment to pharmacy practice via participation in pharmacy technician professional activities and practice advancement accomplishments. Supports improvements in and contributions to pharmacy practice
Leadership and Organizational Involvement
Participates in departmental meetings (75% attendance as scheduled)
Participates in departmental educational activities (100% completion)
Supports practice advancement initiatives, clinical research projects, and stewardship initiatives led by colleagues and peers
Maintains professional image
• Actively participates in a Departmental Practice Council(s) as requested
Qualifications

EDUCATION AND EXPERIENCE REQUIRED:
• One year of Contemporary hospital pharmacy experience
• Bachelor of Pharmacy, Doctor of Pharmacy, or other advanced pharmacy degree
