Job Description Summary

The Clinical Supervisor has 24-hour responsibility for the effective implementation of the philosophy, goals, policies, and procedures of the Home Care Division related to clinical care. The Clinical Supervisor is responsible for the planning, directing, and coordinating all day to day patient care services. The Supervisor is involved and supports the Quality/Performance Improvement initiatives, assists in the Orientation & training of new staff and understands and supports the programs of the Home Health Division.

Job Description


Experience in providing nursing care in the home. Demonstrated competence in home health care. Demonstrated leadership ability and potential managerial competency. Evidence of this includes, but is not limited to, effective communication skills, ability to deal well with people, problem solving, confrontation/resolution issues, motivation of others, and planning, organizing and directing the activities of others. Experience in chronic care management preferable.
