The Intellectual/Developmental Disability Case Management Services Division serves persons diagnosed with an intellectual or developmental disability.

The clinician (case manager) will facilitate the coordination, communication, and collaboration between individuals, families, service providers, and agencies for the purpose of supporting the individual with meeting identified goals and support needs, monitoring services, providing needed assistance, linking to services, and client advocacy.

Essential duties of Clinician II:
  • Develop individual service plans and implement plans in compliance with State Licensure and Medicaid regulations and standards. Ability to conduct thorough assessments, prepare well-written service plans, reports, and progress notes, and maintain individual records in the electronic medical records system in accordance with reviewing authorities' standards and within the established time frames.
  • Assist individuals by linking, locating, monitoring, developing needed services, resources, and benefits, and making collateral contact with the individuals' significant others to promote the implementation of the individual service plan. Ability to identify and utilize community resources, complete benefit applications, and provide assistance if necessary to get the individual to the resource or service provider. Report and record abnormal behaviors, good communication skills and work as a team member with others.
  • Coordinate services with other agencies and providers involved with the individual and maintain necessary records to document the provision of case management services. If the individual's needs require, utilize crisis services, mental health, and medical services.
  • Monitor service delivery and make home and other agency site visits to observe and evaluate individuals' responses to their environment.
Present cases to and participate in WSAC meetings, Prescription Team, assist in conducting preadmission and predischarge planning for state mental retardation facilities. Present information regarding intellectual/developmental disability services to community agencies.

Required Qualifications
VOCATIONAL / EDUCATIONAL REQUIREMENT :
Requires a bachelor's degree in social work, psychology, closely related field, or another degree as approved by the Virginia Department of Health Professions.

EXPERIENCE REQUIREMENT :
In addition to satisfying the vocational/education standards, this position requires a minimum of two years of related, full-time equivalent experience.
  • Depending on assignment, may require Qualified Mental Health Professional ( QMHP - Adult or Child) or Qualified Developmental Disability Professional ( QDDP ) certification and highest Part C standards within 6 months of hire;
  • CPR , First Aid and medication administration certifications within 3 months of hire,
  • Case Management Modules and Intellectual Disabilities Workbook completed within thirty days of hire,
  • Part C Early Intervention Certification within six months of hire, and/or Pre-Screening Certification within 6 weeks of hire (12 weeks for part-time staff).
SPECIAL REQUIREMENT :
Employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies. Emergency operations support work and work locations may be outside of normal job duties.

Preferred Qualifications
QIDP certification preferred.
