Job Description

PURPOSE OF POSITION
Responsible for outreach and staff support

RESPONSIBILITIES


• Plans and participates in targeted Outreach/Public Health Detailing activities in coordination with Site Manager /Outreach Coordinator.


• Functions as a Support staff certifier to support the WIC certification process.


• Point person for the Federally mandated Voter's Registration activity


I. Outreach Worker

• Plans and conducts outreach activities in identified area of need within the catchment area.


• Collaborates with other staff involved with outreach, in coordination with Site Manager.


• Conducts in reach activities in the hospital whenever there is an established need.


• Maintains close working relationship with identified stakeholders of WIC and the Hospital.


• Documents activities in the Outreach Activities Log.


II. Support Staff Certifier

• Ensures that the necessary validation procedures, i.e. checking for positive proof of identification, is followed.


• Reviews Rights and Responsibilities with participants.


• Validates necessary documents required for eligibility determination for income and residency in accordance to NYS policies and procedures.


• Enters into WIC's Information System all required certification data in accordance to NYS policies and procedures.


• Marks on-site and assigns participants to WIC nutritionist following posted daily schedule

(for Certification, for Health and Nutrition Update, for Nutrition Education/ Facilitated Group Discussion)

• Assists in the certification process by conducting on-site hematology and/or obtains blood work information from Hospital Computer or by calling Health Care Provider.


• Assist in conducting on-site anthropometry as needed.


• Conduct program orientation, including explaining program benefits, issuing eWIC card, setting up WIC2GO and providing list of authorized vendors.


• Provide referrals to health and social services, and document in participant's record.


• Issue Farmers Market benefits to participants.


III. Other Responsibilities

• Responsible for the implementation of the federally-mandated Voter Registration activity including reporting requirements and ordering of required forms.


• Coordinates with Systems Analyst and follows up with Help Desk whenever computer issues occur.


• Schedules participant appointments appropriately and alerts supervisor of scheduling concerns or excessive processing timeframes.


• Ensures that work station is neat and well stocked of necessary supplies and materials .


• Demonstrate a professional manner, warmth and respect of participants, to enhance and further the Mission/Vision and Values of Eastside WIC Program.


• Attend staff education as required by the Hospital and NYSD0H.


• Performs other assignments whenever required .


Minimum Qualifications

For Appointment to Level I
1. One year of full-time experience in a government agency or community organization in providing community services to the public or in assisting members of the community in obtaining community services; or
2. Completion of an approved six month training program of combined classroom training and on-the-job experience in community liaison work, plus six months of full-time experience as described in 1 above.

For Appointment to Level II
In addition to meeting the qualification requirements of Assignment Level I, appointment to Level II requires:
1. Two additional years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or
2. Education and/or experience equivalent to 1 above. Study at an accredited college in sociology, psychology or other behavioral science may be substituted on the basis of 30 semester credits for each year of the experience described above. However, all persons must have at least one year of the full-time experience described above.

For Appointment to Level III
In addition to meeting the qualification requirements of Assignment Level II, appointment to Level III requires:
1. One additional year of full-time experience as described in 1 above.

For Appointment to Level IV
In addition to meeting the qualification requirements for Assignment Level III, appointment to Level IV requires:
1. One additional year of full-time experience.
