Overview
Dartmouth-Hitchcock Manchester Nurse Practitioner
The Complex Primary Care Clinic at Dartmouth-Hitchcock is seeking a motivated Nurse Practitioner with a strong interest and background in caring for patients across the lifespan who experience childhood onset illness or disability. The experienced candidate would be joining a Med-Peds physician and APRN, with the opportunity to collaborate with primary care teams and specialists at DHMC, Dartmouth-Hitchcock Manchester, Nashua, and Concord.
The NP would be working in an exclusively outpatient setting, helping to provide and coordinate high-quality care and education to medically complex children and adults and their families as part of the Complex Primary Care clinic. The practice offers a collegial, supportive learning environment. This is a full-time position with no call and limited site travel.
This position allows a variety of responsibilities, including the assessment, diagnosis, and management of complex and acute medical and behavioral health conditions as well as management of chronic medical conditions. The individual selected will act as a patient advocate, assuring that all aspects of the patients' and family's needs are met and fragmentation of care is prevented.
The practice works closely with service providers providing care to many of our patients who receive services administered through the Bureau of Developmental Services.
