Job Description

PURPOSE OF POSITION:

New York City Health + Hospitals has a medical-legal partnership (MLP) with the LegalHealth division of the New York Legal Assistance Group (NYLAG), one of the oldest and largest MLPs in the United States. Through this partnership, thousands of patients are connected to LegalHealth each year with free assistance to address their health-related legal needs. Attorneys also provide support to providers, social workers, and care teams in addressing various social determinants of health.

SUMMARY OF ESSENTIAL DUTIES AND RESPONSIBILITIES:
Under the direction of an Assistant Director, the Coordinator will assist in coordination of all system-wide activities related to patient referrals and appointment scheduling for the LegalHealth program.

General tasks and responsibilities will include:
  • Maintenance of standardized referral and scheduling system for H+H medical-legal appointments
  • Epic coordination for all legal clinic schedules across Health + Hospitals sites
  • Provide coaching and technical assistance to facility staff with questions about patient referrals and legal appointment scheduling
  • Communication with Health + Hospitals facility staff around maintenance and/or changes to legal clinic schedules
  • Management of Epic referral queues across Health + Hospitals locations
    • Monitor volume and data
    • Escalate issues across sites
  • Serve as one of the main patient scheduling contacts for H+H sites without dedicated legal clinic
  • Management of webform referrals from across Health + Hospitals locations
  • Phone calls to referred patients to conduct legal screening, schedule and confirm appointments
  • Coordination of bedside appointments with care team members from various Health + Hospital locations
  • Maintenance of shared inboxes on Outlook
  • Perform other duties and tasks as required or requested by SDOH team leadership


Minimum Qualifications

  1. A Baccalaureate Degree from an accredited college or university in Public Health, Public Administration, Business Administration, Social Work, Psychology or Rehabilitation Counseling, and three (3) years of full-time experience in medically-oriented health care and medical support systems environment, two (2) years of which included experience in an administrative or supervisory capacity in such occupations as physical, occupational, inhalation, speech and audiology therapy or as Medical/Laboratory Technologist, Microbiologist (various specialties), Chemist (biochemist), Radiation and X-Ray Technician or three (3) years of responsible level experience in business management systems or general administration including two (2) years in a supervisory or administrative capacity.
  2. A Masters Degree from an accredited college or university in the disciplines listed in (1), and two (2) years of full-time experience as listed in (1), plus one (1) year experience in an administrative or supervisory capacity in occupations as listed in (1); or,
  3. A satisfactory equivalent combination of training, education and experience.
